จริยา เลาหวิชสไบทิพย์ จูฑะกาญจน์สรางค์รัตน์ เหลืองดำรงชัยอภิญญา วินิตฐานนท์Chariya LaohavichSabaitip ChoothakanSarangrat LaungdamrongchaiApinya Vinithanontมหาวิทยาลัยมหิดล. คณะแพทยศาสตร์โรงพยาบาลรามาธิบดี. ภาควิชาอายุรศาสตร์. หน่วยโรคระบบการหายใจและเวชบำบัดวิกฤต2019-10-252019-10-252562-10-252558รามาธิบดีพยาบาลสาร. ปีที่ 21, ฉบับที่ 1 (ม.ค.-เม.ย. 2558), 1-110858-9739 (Print)2672-9784 (Online)https://repository.li.mahidol.ac.th/handle/123456789/47964อัตราการเลื่อนหรือการงดส่องกล้องตรวจหลอดลม ใน พ.ศ. 2550 สูงถึงร้อยละ 6.32 ทีมหัตถการปอดจึงนำปัญหานี้เข้ามาวิเคราะห์หาสาเหตุ จนได้แนวปฏิบัติในการเตรียมผู้ป่วยก่อน ส่องกล้องตรวจหลอดลม ประกอบด้วย ข้อมูลการเตรียมตัวการทำหัตถการและภาวะแทรกซ้อน ต่างๆ ให้แก่ผู้ป่วยและนำมาสู่การปฏิบัติในปี พ.ศ. 2551 ถึงปี พ.ศ. 2555 ผู้ศึกษาได้ศึกษาข้อมูล ย้อนหลังเพื่อเปรียบเทียบอัตราการเลื่อนและงดส่องกล้องตรวจหลอดลมระหว่างก่อนและหลังใช้ แนวปฏิบัติจากปี พ.ศ. 2551 ถึง พ.ศ. 2555 พบว่า ในปี พ.ศ. 2551 มีจำนวนผู้ป่วยที่มารับบริการ ส่องกล้องตรวจหลอดลมมี 502 ราย มีการเลื่อนและงดส่องกล้องตรวจหลอดลม 21 ราย คิดเป็น ร้อยละ 4.18 ถึงแม้ว่าจำนวนผู้มารับบริการส่องกล้องจะมากขึ้นเรื่อยๆ แต่หลังจากเริ่มใช้แนวปฏิบัติ ในการเตรียมผู้ป่วยส่องกล้องตรวจหลอดลม อัตราการเลื่อนและงดส่องกล้องลดลงทุกๆ ปี ลดเหลือร้อยละ 1.12 ในปี พ.ศ. 2555 และยังคงมีการใช้และมีการพัฒนาแนวปฏิบัติเพื่อลดอัตรา การเลื่อนและงดส่องกล้องตรวจหลอดลมอย่างต่อเนื่องIn 2007, appointment postponement and cancellation rate in patients undergoing bronchoscopy was high as 6.32%. Therefore, we had a conference among the bronchoscopy team to explore the related factors of this problem. Then, the work instructions were developed and used to improve the appointment systems, including full explanation of the procedure, patients’ preparation, and complications to the patients. This system had been used into practice from 2008 through 2012 with continuing evaluation. This retrospective study was conducted to compare the rates of appointment postponement and cancellation between before and after using work instruction from 2008 to 2012. Out of 502 procedures, appointment postponement and cancellation occurred in 21 procedures (4.18%) in 2008. Although the number of bronchoscopic procedure in this institute increased year by year, the appointment postponement and cancellation decreased after starting work instruction for bronchoscopy appointment. The event frequency went down to 1.12% in 2012.
