ศิริพร คล้ายทิมพิมพ์ณดา พิชัยภาณุพัฒน์Siriporn KhlaiythimPimnada Pichaiphanupatมหาวิทยาลัยมหิดล. คณะแพทยศาสตร์โรงพยาบาลรามาธิบดี. ฝ่ายการพยาบาล. งานการพยาบาลผ่าตัด2019-12-172019-12-172562-12-172555รามาธิบดีพยาบาลสาร. ปีที่ 18, ฉบับที่ 1 (ม.ค.-เม.ย. 2555), 1-80858-9739 (Print)2672-9784 (Online)https://repository.li.mahidol.ac.th/handle/20.500.14594/48422อุปกรณ์ปกป้องเนื้อเยื่อรามาธิบดี (Ramathibodi soft tissue protector) เป็นอุปกรณ์ ที่ใส่เข้าไปในช่องระหว่างกระดูกซี่โครงในการผ่าตัดลิ้นหัวใจแบบแผลเล็ก (minimally invasive mitral valve procedure) และการผ่าตัดปอดผ่านกล้อง (video assisted thoracic surgery) เพื่อช่วยลดการบอบชํ้าของเนื้อเยื่อจากเครื่องมือที่ใช้ในการผ่าตัด เครื่องจี้ไฟฟ้า และไหมเย็บ อุปกรณ์ปกป้องเนื้อเยื่อรามาธิบดีดัดแปลงและประดิษฐ์มาจากวัสดุอุปกรณ์ที่หาได้ในห้องผ่าตัด ทำได้ง่าย ราคาย่อมเยา ใน 1 ชุด ประกอบด้วยห่วงยางซิลิโคนรูปวงกลม 2 อัน Stockinette stitch ขนาดเส้นผ่าศูนย์กลาง 8 เซนติเมตร ยาว 13 เซนติเมตร และด้ายเย็บผ้าที่ใช้เย็บ Stockinette stitch ติดกับห่วงยางซิลิโคนทั้ง 2 ปลาย อุปกรณ์ปกป้องเนื้อเยื่อรามาธิบดีมีคุณลักษณะที่เป็น ข้อดีกว่าอุปกรณ์ที่นำเข้าจากต่างประเทศคือ มีความอ่อนนุ่ม ยืดหยุ่น ไม่ฉีกขาดขณะทำผ่าตัด มีขนาด พอดีกับขนาดของช่องระหว่างกระดูกซี่โครง และมีราคาถูก เริ่มใช้ในห้องผ่าตัดศูนย์การแพทย์สิริกิติ์ หน่วยหัวใจและทรวงอก ภาควิชาศัลยศาสตร์ คณะแพทยศาสตร์โรงพยาบาลรามาธิบดี ตั้งแต่ เดือนสิงหาคม 2553 จนถึงเดือนกรกฎาคม 2554 จากการรวบรวมสถิติในผู้ป่วยจำนวน 30 รายไม่พบภาวะแทรกซ้อนจากการใช้อุปกรณ์ปกป้องเนื้อเยื่อรามาธิบดี อุปกรณ์ปกป้องเนื้อเยื่อ รามาธิบดีสามารถนำไปประยุกต์ใช้ในคลินิก ปรับปรุงและพัฒนาการผ่าตัดลิ้นหัวใจและปอดแบบ แผลเล็กได้เป็นอย่างดี ช่วยให้ผู้ป่วยประหยัดค่าใช้จ่ายในการผ่าตัดRamathibodi soft tissue protector is a device used to insert into intercostal space in a minimally invasive mitral valve procedure (MV procedure) and video assisted thoracic surgery (VATS) to reduce tissue injury from instruments, electrocuatery, and sutures. The Ramathibodi soft tissue protector is modified and invented from the material simply found in the operating room. The inventing process is simple and inexpensive. A set of the Ramathibodi soft tissue protector consists of two silicone rings, a stockinette stitch with 8 centimeters in diameter and 13 centimeters in length and sewing thread. The Ramathibodi soft tissue protector has more advantageous characteristics over the commercially imported protector in terms of its softness, elasticity (not tearing during operation), shape fitting to the intercostals, and low cost. These characteristics can improve the surgical techniques. In clinical use with 30 patients who underwent the MV procedure and VATS at Faculty of Medicine Ramathibodi Hospital, no protector-related complication has been found from August 2010 to July 2011.
