The major advantage of this procedure is to minimize the risk of tumor cell contamination, lower local recurrent rate, improve survival time and quality of life for these particular patients. However, with respect to the surgical technique, TES requires the special instruments that are not available in Thailand. Thus, we need to adapt the surgical techniques for the success in this procedure. We succeed the TES procedure without using the special instruments. Therefore, the objectives of this report is to illustrate our modifications of the original surgical technique to achieve the successful TES procedure using general standard surgical equipments. The steps of our approach will be discussed in the detail. However, careful selection of patients, considering of the underlying tumor type and its extension are essential for the successful outcome.การผ่าตัดกระดูกสันหลังออกทั้งกลุ่ม (Total en bloc spondylectomy: TES) มีข้อบ่งชี้ในการรักษาโรคมะเร็งกระดูกสันหลังชนิดร้ายแรง (Primary malignant neoplasm) หรือมะเร็งระยะลุกลามที่มีการแพร่กระจายมายังกระดูกสันหลังปล้องเดี่ยว (Solitary spinal metastasis) การรักษาด้วยวิธีนี้มีข้อได้เปรียบคือ สามารถลดการปนเปื้อนของเซลล์มะเร็งจากการผ่าตัด จึงสามารถลดอุบัติการณ์การเกิดซ้ำของมะเร็ง และยังสามารถยืดระยะเวลาการมีชีวิต (Survival time) ของผู้ป่วยมะเร็งระยะลุกลาม ที่มีการแพร่กระจายมายังกระดูกสันหลังได้โดยมีคุณภาพชีวิตที่ดีขึ้น อย่างไรก็ตาม การผ่าตัดโดยวิธีดังกล่าวต้องการเครื่องมือพิเศษที่ไม่สามารถหาได้ในเมืองไทยคณะผู้วิจัยจึงได้คิดค้นวิธีผ่าตัดโดยประยุกต์เพื่อที่จะสามารถทำผ่าตัดด้วยวิธีดังกล่าวได้โดยไม่ต้องใช้เครื่องมือพิเศษเพิ่มเติม วิธีและขั้นตอนการผ่าตัดจะได้กล่าวในบทความโดยละเอียด อย่างไรก็ตาม ความสำเร็จของการรักษานั้นขึ้นอยู่กับการเลือกผู้ป่วยที่เหมาะสม โดยพิจารณาถึงชนิดและการลุกลามของมะเร็งเป็นสำคัญengMahidol UniversityTotal En Bloc Spondylectom Solitary spinal metastasisการผ่าตัดกระดูกสันหลังการประยุกต์วิธีการผ่าตัดOriginal ArticleDepartment of Orthopedics Faculty of Medicine Ramathibodi Hospital Mahidol UniversityOrthopedic Division Banmi Hospital
