ธัญวรัตน์ ทองเงินศริยามน ติรพัฒน์ยุวนุช สัตยสมบูรณ์Thanwarat ThongngunSariyamon TiraphatYouwanuch Sattayasomboon2024-06-262024-06-262567-06-262567วารสารกฎหมายและนโยบายสาธารณสุข. ปีที่ 10, ฉบับที่ 2 (พ.ค.-ส.ค. 2567), 329-3462697-6285 (Online)https://repository.li.mahidol.ac.th/handle/20.500.14594/99030การศึกษาครั้งนี้ เป็นการวิจัยเชิงพรรณนาแบบภาคตัดขวาง วัตถุประสงค์เพื่อประเมินระดับการดำเนินงานการดูแลผู้สูงอายุที่มีภาวะพึ่งพิง วิเคราะห์ปัจจัยที่มีความสัมพันธ์และมีอิทธิพลต่อการดำเนินงานการดูแลผู้สูงอายุที่มีภาวะพึ่งพิงของผู้จัดการระบบการดูแลระยะยาวด้านสาธารณสุข จังหวัดประจวบคีรีขันธ์ ตัวแปรอิสระ ได้แก่ ปัจจัยส่วนบุคคล และปัจจัยการบริหาร กลุ่มตัวอย่าง คือ ผู้จัดการระบบการดูแลระยะยาวด้านสาธารณสุข จังหวัดประจวบคีรีขันธ์ จำนวน 131 คน เครื่องมือการวิจัย คือ แบบสอบถาม ซึ่งผ่านการตรวจสอบคุณภาพเครื่องมือจากผู้ทรงคุณวุฒิ จำนวน 3 ท่าน โดยการหาดัชนีความตรงเชิงเนื้อหา และความเชื่อมั่นของเครื่องมือ โดยมีระยะเวลาในการเก็บรวบรวมข้อมูล ตั้งแต่วันที่ 3 – 17 มกราคม พ.ศ. 2567 วิเคราะห์ข้อมูลโดยการหาค่าความถี่ ร้อยละ ค่าเฉลี่ย สถิติไคสแควร์ และสถิติการถดถอยโลจิสติกทวิ ผลการศึกษาพบว่า ภาพรวมผู้จัดการระบบการดูแลระยะยาวด้านสาธารณสุขมีระดับการบริหารและระดับการดำเนินงานการดูแลผู้สูงอายุที่มีภาวะพึ่งพิงอยู่ในระดับสูง (ร้อยละ 50.4, 61.80) ตามลำดับ ผลการวิเคราะห์สถิติไคสแควร์ ชี้ให้เห็นว่าปัจจัยที่มีความสัมพันธ์กับการดำเนินงานการดูแลผู้สูงอายุที่มีภาวะพึ่งพิง อย่างมีนัยสำคัญทางสถิติ ได้แก่ ปัจจัยส่วนบุคคลด้านรายได้เฉลี่ยต่อเดือน ปัจจัยการบริหารด้านบุคลากร ด้านงบประมาณ และด้านบริหารจัดการ ส่วนผลการวิเคราะห์สถิติการถดถอยโลจิสติกทวิ พบว่าในมิติด้านบริหารจัดการมีผลต่อการดำเนินงานการดูแลผู้สูงอายุที่มีภาวะพึ่งพิงของผู้จัดการระบบการดูแลระยะยาวด้านสาธารณสุข จังหวัดประจวบคีรีขันธ์ อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 ผลที่ได้รับจากการวิจัยเป็นหลักฐานเชิงประจักษ์เพื่อให้ผู้กำหนดนโยบายนำไปใช้ในการวางแผน กำหนดนโยบายหรือแผนการดำเนินงานในการปรับปรุงวิธีการปฏิบัติงาน และแก้ไขปัญหาอุปสรรค เพื่อเป็นแนวทางพัฒนาระบบการดูแลระยะยาวด้านสาธารณสุขสำหรับผู้สูงอายุที่มีภาวะพึ่งพิงอย่างเหมาะสมต่อไปThis research is a cross-sectional analytical study aimed at evaluating the level of operation, analyzing the relationship and the effecting factors and administrative factors with the operation of the public health long-term care system toward dependent older adults in Prachuap Khiri Khan Province, Thailand. The sample group consists of 131 long-term care system managers in the public health sector in Prachuap Khiri Khan Province. The research tool is a questionnaire that was examined and verified by the three experts for content validity index and tested for reliability. Data collection took place from January 3-17, 2024. Data analysis was conducted for frequency, percentage, mean, Chi-square test, and Binary logistic regression analysis. The study found that care managers have high management and operational performance levels in caring for dependent older adults, with 50.4% and 61.80%, respectively. Statistical analysis using chi-square tests indicates that factors significantly related to the care management of dependent older adults include income, personnel management, budget, and administrative management. Furthermore, statistical logistic regression analysis revealed that the dimension of administrative management significantly affects the operational performance in caring for dependent older adults by long-term public health system managers in Prachuap Khiri Khan Province, Thailand, at a statistical significance level of 0.05. The research findings serve as empirical evidence for policymakers to use in planning, improving operational practices, and addressing challenges.
