Depending on the severity and the type of the curve, its effects on the movement of the rib cage during respiration are varied. It was also found that the reduction of pulmonary function is dependent on the type of the curve and the duration of the bracing. Long-term bracing may impair pulmonary function, and braces can also mimic restrictive pulmonary disease. Therefore, it is important to consider pulmonary functions in the conservative treatment of AIS. Other studies have also reported that AIS affects the functional capacity and quality of life of patients. Thus, the aim of the study is to investigate the effects of the Cheneau and custom-made Boston braces on pulmonary function, energy consumption, and quality of life in AIS. The Cheneau brace is structured as an asymmetric brace, while the custom-made Boston brace is symmetric. Tissue migration and the accommodation of expanded lungs can be achieved inside the Cheneau brace, which also intertwines with the principles of Schroth's corrective breathing techniques. Therefore, the structural and biomechanical differences between the two brace designs might lead to differences in pulmonary function. Four participants—two Cheneau brace users and two Boston brace users who had been using their braces for six months—were recruited for the study, and data were collected two times at one-year intervals. The Cortex Meta Max (3B-R2) was used for collecting spirometry and oxygen consumption data, while the Modified Borg’s Dyspnea scale and SRS-22 questionnaires were used as patient-reported outcome measurements. In conclusion, the custom-made Boston brace user group showed a lower reduction of pulmonary function, lower gait oxygen consumption, and better quality of life compared to the Cheneau brace users, although they reported lower scores in satisfaction and management with the custom-made Boston braces. The level of exertion and resting minute ventilation were similar between the two groups. However, all parameters in both groups showed a reduction in in-braced conditions when compared to out-braced data. As a result, the custom-made Boston brace showed better results in terms of pulmonary function, energy expenditure, and quality of life compared to the Cheneau brace. Implication of Thesis: This study can help clinicians and researchers in decision-making for prescription and treatment plans for adolescent idiopathic patients. It can also help clinicians to understand changes in pulmonary function, energy consumption, and quality of life of AIS patients based on Cheneau and custom-made Boston braces over time, helping to improve scoliosis management plans in the future.
