A 77-year-old immunocompetent man was clinically diagnosed with right malignant otitis externa. He had been receiving combined antibiotics for five months for another illness. No etiologic agent was identified from aural discharge culture. Tympanomastoidectomy was done after no response was seen to empirical antibiotics.
