อรวิธู กาญจนจารียุพาพิน ศิรโพธิ์งามสุปรีดา มั่นคงOrnwitoo KanchanachariYupapin Sirapo-ngamSupreeda Monkongมหาวิทยาลัยมหิดล. คณะแพทยศาสตร์โรงพยาบาลรามาธิบดี. โรงเรียนพยาบาลรามาธิบดี2019-10-222019-10-222562-10-222560รามาธิบดีพยาบาลสาร. ปีที่ 23, ฉบับที่ 3 (ก.ย.-ธ.ค. 2560), 328-3430858-9739 (Print)2672-9784 (Online)https://repository.li.mahidol.ac.th/handle/20.500.14594/47945การศึกษาเชิงคุณภาพนี้มีวัตถุประสงค์เพื่อศึกษาบทบาทและความต้องการของผู้ดูแล ผู้ป่วยระยะท้ายในสถานบริการที่มีการดูแลแบบประคับประคองโดยเฉพาะ คัดเลือกผู้ดูแลหลัก ทั้งหญิงและชายที่อายุ20 ปีขึ้นไป ดูแลช่วยเหลือผู้ป่วยระยะท้ายก่อนและระหว่างมารับบริการที่ โรงพยาบาลมหาวชิราลงกรณธัญบุรีจำนวน 9 รายเก็บรวบรวมข้อมูลจากการสนทนากลุ่มจำนวน 2 กลุ่มกลุ่มละ 4-5 ราย และสัมภาษณ์เดี่ยว 4 รายโดยใช้แบบสอบถามและแนวทางการสัมภาษณ์ บทบาทและความต้องการของผู้ดูแลผู้ป่วยระยะท้ายวิเคราะห์ข้อมูลเชิงเนื้อหา ผลการศึกษาพบว่า บทบาทของผู้ดูแลผู้ป่วยระยะท้ายประกอบด้วย 3 บทบาท ได้แก่ 1) การดูแลผู้ป่วยโดยตรงด้าน ร่างกายจิตใจสังคม และจิตวิญญาณ 2) การเตรียมพร้อมสำหรับการดูแลระยะท้าย และ 3) การ ปรับตัวและการดูแลตนเอง ส่วนความต้องการของผู้ดูแลผู้ป่วยในระยะท้าย ประกอบด้วย 4 ประเด็นได้แก่1) ความต้องการข้อมูลในการจัดการดูแลผู้ป่วยระยะท้าย2)ความต้องการประสานงาน เพื่อเข้าถึงบริการระยะท้าย 3)ความต้องการให้บุคลากรของสถานบริการช่วยดูแลผู้ป่วยระยะท้าย แทนตนเองเมื่อจำเป็น และ 4) ความต้องการความช่วยเหลือด้านการเงิน ข้อค้นพบนี้สามารถนำ ไปใช้เป็นข้อมูลพื้นฐานในการออกแบบแนวทางในการให้ความช่วยเหลือผู้ดูแลผู้ป่วยระยะท้ายที่ เข้ารับบริการในสถานบริการที่มีการดูแลแบบประคับประคองโดยเฉพาะThis study aimed to describe roles and needs of caregivers for patients with end of life in a specific setting for palliative care. The participants included 9 primary caregivers both female and male aged over 20 years who took the majority of care for end of life patients before and during palliative care service at Mahavajiralongkorn Thanyaburi Hospital. Data were collected by conducting two focus groups and then performing in-depth interview in 4 individuals by using questionnaire and interview guidelines on roles and needs of caregivers for end of life patients. Data were analyzed using content analysis. The results indicated that there were three major roles of caregiver: 1) direct patient care which included physical, psychological, social, and spiritual; 2) preparation for the end of life care; and 3) role adaptation along with self-care of their own. Caregivers’ needs were composed of four aspects which were: 1) information need about end of life care management; 2) coordination need for accessing to end of life care; 3) helping need for patient care; and 4) financial support need.
