Therefore, the pharmaceutical care by pharmacist was provided in order to increase the effectiveness of treatment. The process starts from the following: getting a patient’s medical history, participating in the assessment of disease severity for medication selection, counseling the physician about the appropriate medicine, and giving patient counselling for the proper use of medication.
