Department of Operative Dentistry and EndodonticsObjective: To present the usefulness of cone-beam computerized tomography (CBCT) in order to judge the necessity of an intentional endodontic treatment before crown preparation in an over-erupted tooth. Materials and methods: The upper left first molar was over-erupted 2.5 to 3 mm from occlusal plane and planned to be adjusted before fabricating the new dentures. From the preparation on duplicated model, 4.0-4.5 and 3.5-4.0 mm preparation depth were needed at the buccal cusps and palatal cusps. CBCT was used to obtain three-dimensional details of tooth. Results: From the CBCT images, the highest points of pulpal horns were clearly demonstrated. The minimal remaining tooth thicknesses obtained from CBCT were 3.25, 3.5 and 4.0 mm for mesiobuccal, distobuccal and palatal pulpal horns, respectively. Since the pulpal exposure would occur during crown preparation, intentional endodontic therapy was initiated. Conclusion: CBCT is an alternative radiographic option before initiating intentional endodontic treatment.
