    Development of a motivational interviewing program in diet control based on the context of diabetics

    The purposes of this study were to 1) developed a motivational interviewing program to strengthen motivation for diet control, and 2) to study the effectiveness and explain the mechanism of motivation change in diabetic patients. This developmental study was divided into 2 phases. In phase 1, 5 case studies of diabetic patients were conducted by 1) analysing readiness and level of diet control motivation, 2) developing program formats, 3) testing the program trial and improving it for the second phase and Phase 2 of the study, 8 case studies of diabetic patients, was conducted for evaluation of the change of readiness in diet control and exploration on the change of motivation process on diet control in the social context. The results of the phase 1 showed that the implementation of the motivational interviewing program should begin with a survey of the readiness and level of motivation to change behavior, and other background contexts of the patients that might influence their motivation. This would help the coordinator know the thoughts and feelings of their patients towards diabetes control, and enable to use this findings to determine how to have appropriate communication with the diabetic patients. The second phase of the study found that the improved program showed more effects on the patients’ readiness and could increase motivation of diet control. In this phase, the patients were found to speak the Self-motivation Statement (SMS) that expressed their intention to change their diet control behavior. The mechanisms that influenced the change in readiness for behavioral adjustment and the level of motivation in dietary control of the diabetic patients were the skills and techniques used in the motivational interviewing program. This was the result of collaboration between the program coordinator and the patients, which provided freedom in making decisions about their own behavior and also allowed the patients to receive basic psychological needs such as independence in determining behavior, as well as encouragement to be aware of their own ability to act and behave according to their own context. Therefore, the format of a motivational interviewing program for diet control in diabetic patients should focus on the patients’ readiness for behavioral change and their autonomy in accordance with their background context.
