การศึกษาความคิดเห็นของพยาบาลที่มีต่อการใช้แนวทางการป้องกันและดูแลรักษาผู้ป่วยโดยกำหนดตัวส่งสัญญาณแต่เนิ่น ๆ ในโรงพยาบาลศิริราช

Abstract

การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาความคิดเห็นของพยาบาลที่มีต่อการใช้ Siriraj Concurrent Trigger Tool by Modified Early Warning Signs (SiCTT by MEWS) เพื่อความปลอดภัยของผู้ป่วย กลุ่มตัวอย่างคือ พยาบาลที่ปฏิบัติงานในหอผู้ป่วย โรงพยาบาลศิริราช อายุงานมากกว่า 3 ปี จำนวน 465 คน เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามซึ่งมีค่าสัมประสิทธิ์แอลฟา เท่ากับ 0.916 สถิติที่ใช้วิเคราะห์ข้อมูล ได้แก่ สถิติเชิงบรรยาย และ การทดสอบค่าที ผลการศึกษาพบว่ากลุ่มตัวอย่างส่วนใหญ่เป็นเพศหญิง (ร้อยละ 96.5) อายุเฉลี่ย 36.07±9.094 ปี (พิสัย 22-59 ปี) อายุงานเฉลี่ย 13.49±9.109 ปี (พิสัย 3-48 ปี) หน่วยงานที่ตอบแบบสอบถามมากที่สุดคือ งานการพยาบาลอายุรศาสตร์ฯ (ร้อยละ 30.8) เป็นพยาบาลวิชาชีพระดับปฏิบัติการมากที่สุด (ร้อยละ 46.8) การศึกษาระดับปริญญาตรี (ร้อยละ 80.5) ส่วนใหญ่เป็นผู้ใช้แนวปฏิบัติ (ร้อยละ 93.8) ระยะเวลาในการใช้แนวปฏิบัติอยู่ระหว่าง 1-120 เดือน (เฉลี่ย 30.70±16.65 เดือน) จำนวนแนวปฏิบัติที่ใช้อยู่ระหว่าง 1-9 เรื่องต่อคน (เฉลี่ย 4.45±2.08 เรื่อง) แนวปฏิบัติ SiCTT by MEWS ที่ใช้มากที่สุด คือ แนวทางปฏิบัติในการดูแลรักษาผู้ป่วยผู้ใหญ่ที่ได้รับ Heparin (ร้อยละ 69.5) ความคิดเห็นในการใช้ SiCTT by MEWS พบว่าระดับความปลอดภัยของผู้ป่วยก่อนและหลังใช้ SiCTT by MEWS มีความแตกต่างอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 และเมื่อวิเคราะห์ความคิดเห็นที่มีต่อระดับความปลอดภัยของผู้ป่วย ระหว่างกลุ่มผู้ใช้ และผู้พัฒนาแนวปฏิบัติ (ผู้มีส่วนร่วมในการสร้าง/พัฒนา และผู้ให้ข้อเสนอแนะ) ทั้งภาพรวมและรายด้าน พบว่าทั้ง 2 กลุ่มไม่มีความแตกต่างอย่างมีนัยสำคัญทางสถิติที่ 0.05 สรุปได้ว่าความคิดเห็นของพยาบาลที่ใช้แนวปฏิบัติที่พัฒนา SiCTT by MEWS เห็นด้วยว่า มีผลต่อความปลอดภัยของผู้ป่วยมากขึ้น โดยมีข้อเสนอแนะเพิ่มเติมว่าควรมีการพัฒนาเรื่องกระบวนการดำเนินงาน และการสื่อสารนโยบายและความสำคัญของการใช้ SiCTT by MEWS ให้ชัดเจน เพื่อให้เกิดการดำเนินงานที่เป็นระบบและมีประสิทธิภาพส่งผลให้ผู้ป่วยเกิดความปลอดภัยสูงสุด

The purpose of this study was to examine the opinions of nurses on using the Siriraj Concurrent Trigger Tool by Modified Early Warning Signs (SiCTT by MEWS) nd its impact on patient safety. The sample was drawn from nurses working at the Inpatient Department, Siriraj Hospital, and it comprised 465 nurses with at least 3 years’ work experience. Data was collected by using a questionnaire with an alpha coefficient (α-coefficient) of 0.916. Data were analyzed using descriptive statistics and t-test. The result found that the majority of the sample group were female (96.5%), with an average age of 36.07 ± 9.09 years (range 22–59 years). Their average working life was 13.49±9.11 years (range 3–48 years). Most of the nurses were involved in internal medicine nursing (30.8%). The highest educational level was found among professional nurses (46.8%), who typically had a bachelor's degree (80.5%); most nurses in this subgroup were users of SiCTT by MEWS (93.8%). The practice is between 1–120 months (average 30.70 ± 16.65 months). The number of SiCTT by MEWS usage between 1-9 stories per person (average 4.45 ± 2.08 subjects). SiCTT by MEWS was reported to be the most commonly used practice guideline for the care of adult patients who received heparin (69.5%). The nurses’ opinions of SiCTT by MEWS was a statistically significant difference in the safety level before and after using SiCTT by MEWS (p = 0.05). And when analyzing opinions on patient safety levels between user groups and the guideline developer (participants in the creation/development suggestions and recommendations), both in general and in each aspect, found that groups were not significantly different at 0.05. Conclusions: There was agreement among the nurses who used SiCTT by MEWS that it improved the safety of patients. With the development of operational processes and clear communication of the policy and importance of using SiCTT by MEWS to ensure systematic and effective operations resulting in maximum safety for patients.
