การสังเคราะห์องค์ความรู้เกี่ยวกับการป้องกันและจัดการการหกล้มในผู้ป่วยที่รับไว้รักษาในโรงพยาบาล

Abstract

การหกล้มเป็นปัญหาสำคัญของผู้ป่วยที่รับไว้รักษาในโรงพยาบาล ซึ่งส่งผลกระทบต่อ ผู้ป่วยทั้งด้านร่างกายและจิตใจ รวมถึงบุคลากรและองค์กร การหกล้มเป็นตัวชี้วัดคุณภาพที่เป็น ภาพรวมของโรงพยาบาลในเชิงผลลัพธ์ทางการพยาบาลและส่งเสริมความปลอดภัย การศึกษา ครั้งนี้มีวัตถุประสงค์เพื่อทบทวนหลักฐานทางวิชาการที่เกี่ยวกับการป้องกันและจัดการการ หกล้มในผู้ป่วยที่รับไว้รักษาในโรงพยาบาล โดยสืบค้นจากแหล่งข้อมูลตั้งแต่ปี พ.ศ.2533-2550 เป็นงานวิจัยจำนวน 16 เรื่อง จัดอยู่ในระดับ A 4 เรื่อง ระดับ B 10 เรื่อง และระดับ C 2 เรื่อง ผลการศึกษา จำแนกเนื้อหาออกเป็น 3 ส่วนคือ 1) การประเมินปัจจัยเสี่ยงต่อการหกล้ม ประกอบด้วยปัจจัยภายใน และปัจจัยภายนอก เครื่องมือที่เป็นมาตรฐานในการประเมินปัจจัยเสี่ยง และใช้มากที่สุดคือ Morse Fall Scale 2) การบำบัดทางการพยาบาลเพื่อป้องกันและจัดการการ หกล้มแบ่งได้เป็น 2 ประเภทคือ การป้องกันการหกล้มแบบที่มุ่งบำบัดเพียงปัจจัยเดียว และ การป้องกันการหกล้มที่มุ่งบำบัดหลายปัจจัยร่วมกัน ผลการศึกษา พบว่า การป้องกันการหกล้มที่ มุ่งบำบัดหลายปัจจัยร่วมกัน สามารถป้องกันและลดอัตราการหกล้มได้อย่างมีนัยสำคัญ และ 3) ระบบการประเมินปัจจัยเสี่ยงที่เป็นมาตรฐานในระดับองค์กร เพื่อป้องกันและจัดการการหกล้ม ในผู้ป่วยที่รับไว้รักษาในโรงพยาบาล ผลการศึกษาครั้งนี้พบว่าการหกล้มสามารถป้องกันได้ องค์ ความรู้ที่ได้สามารถนำไปใช้ประโยชน์ในการวางแผนการป้องกันการหกล้มในผู้ป่วยที่รับไว้รักษา ในโรงพยาบาล และเป็นการปฏิบัติโดยอาศัยหลักฐานเชิงประจักษ์และก่อให้เกิดประสิทธิผล รวมทั้ง เป็นแนวทางในการพัฒนาแนวปฏิบัติในการป้องกันและจัดการการหกล้มในผู้ป่วยที่รับไว้รักษา ในโรงพยาบาล

Falls are common problems for hospitalized patients and they affect patients both physically and mentally. They also affect health care providers and organizations. Falls are a nursing indicator of a hospital’s efficacy in nursing outcomes and safety promotion. A literature review on fall prevention and management of hospitalized patients was the purpose of this study. Sixteen research studies published between 1990 and 2007 were used for quality evaluation. Of the research studies, four were classified as level A, ten as level B, and two as level C. The research findings were classified to three different categories. The first category consisted of evaluation of fall risk factors which consisted of intrinsic factors and extrinsic factors; the Morse Fall Scale is the most popular one. The second category dealt with nursing interventions for fall prevention and management in hospitalized patients through evidence-based practice and for the establishment of nursing practice guidelines. They were classified into two types: 1) unifactorial interventions in which no prevention or reduction of the fall rate was achieved; 2) multifactorial interventions which achieved significant preventions and reduction of the fall rate. The third category dealt with the standard of the fall risk assessment system in the organization to prevent and manage falls in hospitalized patients. The results of this study support the notion that falls are preventable. The knowledge gained from this study could be beneficial for fall prevention and management in hospitalized patients, and for development of clinical practice guidelines for this population.
