แนวโน้มความกว้างฟันของผู้อาศัยในกรุงเทพมหานคร

Abstract

วัตถุประสงค์ : วัตถุประสงค์ของการศึกษาคือเพื่อศึกษาความกว้างของฟันในคนกรุงเทพมหานครตั้งแต่พุทธศักราช 2515 ถึง 2551 ทันตแพทย์โดยเฉพาะอย่างยิ่งทันตแพทย์สาขาทันตกรรมจัดฟันและทันตกรรมประดิษฐ์มีความจำเป็นต้องใช้ความกว้างฟันเฉลี่ยที่ได้มาตรฐาน มนุษย์ในยุคใหม่มีแนวโน้มชัดเจนว่ามีความกว้างของฟันแคบลงตามหลักวิวัฒนาการ อีกทั้งมีปัจจัยหลายประการทั้งจากพันธุกรรมและสิ่งแวดล้อมที่ส่งผลต่อความกว้างฟัน ด้วยเหตุนี้ จึงเป็นสิ่งน่าสนใจที่จะศึกษาแนวโน้มการเปลี่ยนแปลงความกว้างฟัน หากพบว่ามีแนวโน้มการลดลงของความกว้างฟัน เหตุผลนี้ส่งผลให้มีความจำเป็นที่จะต้องศึกษาความกว้างฟันซ้ำเป็นระยะๆ วัสดุอุปกรณ์และวิธีการศึกษา : การศึกษานี้วัดความกว้างฟันจากแบบจำลองฟันที่มีการสบฟันดีของคนกรุงเทพมหานครจำนวน 67 คู่ โดยเริ่มพิมพ์ฟันผู้ป่วยในปีพุทธศักราช 2544 จากผู้เข้าร่วมวิจัยที่มีสุขภาพแข็งแรง เป็นเพศชาย 16 คน และเพศหญิง 51 คน (n=67) จากนั้นทดสอบความกว้างของฟันว่ามีความแตกต่างระหว่างเพศหรือไม่ และนำความกว้างฟันนนี้มาเปรียบเทียบกับความกว้างฟันในการศึกษาที่มีการบันทึกไว้ ผลการศึกษา : จากผลการศึกษาพบว่าฟันเกือบทุกซี่ไม่มีความแตกต่างอย่างมีนัยสำคัญทางสถิติของความกว้างฟันของเพศชายและหญิง ยกเว้นฟันเขี้ยวล่างซ้าย ค่าความกว้างฟันของการศึกษานี้แตกต่างจากการศึกษาก่อนหน้าโดยส่วนใหญ่ อย่างไรก็ตามไม่สามารถตรวจพบแนวโน้มการมีความกว้างฟันลดลงในระยะเวลา 36 ปี บทสรุป : โดยส่วนใหญ่พบว่ามีความแตกต่างระหว่างความกว้างฟันของการศึกษานี้เมื่อเทียบกับการศึกษาอื่นๆ ถึงแม้ว่าการหาค่าเฉลี่ยความกว้างฟันเป็นระยะๆ จะเป็นแนวทางที่แม่นยำ การศึกษานี้ไม่สามารถตรวจพบแนวโน้มการแคบลงความกว้างฟัน

Objective: The objective of this study was to investigate the mesio-distal tooth width of Bangkok residents from 1972 to 2008. Standardized average tooth width is essential for dentists, especially orthodontists and prosthodontists. Modern human showed strong tendency of tooth width reduction due to evolution. Many factors also affecting tooth width include genetic and environmental factors. Due to this change, it is interesting to investigate the secular trend of tooth widths. Any significant reduction of tooth width certainly needs to be reinvestigated periodically. Materials and Methods: In 2011, the 67 pairs of dental models with good occlusion of Bangkokians were investigated for tooth width. These models were taken from healthy subjects, 16 males and 51 females (n=67). These tooth widths were tested for sex related differences. Then these tooth width measurements again were compared with documented studies. Results: The difference of tooth width of this study due to sex was not statistically significant except for the mandibular left canine. The results also showed statistically significant differences between tooth width of this study and most other studies. However, the tendency of tooth width reduction over 36 years could not be established. Conclusion: Significant difference of tooth width was found among different groups investigated. Although it would be more accurate to reinvestigate tooth width periodically, this study could not show the reduction tendency of the tooth width.
