ความวิตกกังวลและภาวะซึมเศร้าในหญิงตั้งครรภ์ที่มีภาวะเจ็บครรภ์ก่อนกำหนดที่ห้องคลอด โรงพยาบาลรามาธิบดี

Abstract

การวิจัยเชิงบรรยายครั้งนี้เป็นการศึกษาไปข้างหน้า มีวัตถุประสงค์เพื่อศึกษา 1) อัตราและระดับความวิตกกังวลและภาวะซึมเศร้าของหญิงตั้งครรภ์ที่มีภาวะเจ็บครรภ์คลอดก่อนกำหนด 2) ความสัมพันธ์ระหว่างปัจจัยส่วนบุคคล ได้แก่ อายุ ระดับการศึกษา อาชีพ รายได้ครอบครัว กับความวิตกกังวลและภาวะซึมเศร้าในหญิงตั้งครรภ์ที่มีภาวะเจ็บครรภ์คลอดก่อนกำหนด กลุ่มตัวอย่างเป็นหญิงตั้งครรภ์ที่มีอายุครรภ์ระหว่าง 24 - 36 สัปดาห์ที่ได้รับการวินิจฉัยว่ามีภาวะเจ็บครรภ์ก่อนกำหนด และรับไว้รักษาในโรงพยาบาลรามาธิบดีระหว่างเดือนกันยายน 2556 ถึงเดือนมีนาคม 2557 จำนวน 60 ราย เลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง (purposive sampling) เก็บข้อมูลภายใน 24 ชั่วโมงโดยใช้ 1. แบบสอบถามข้อมูลส่วนบุคคล 2. แบบสอบถามความวิตกกังวลและภาวะซึมเศร้าขณะเข้ารับการรักษาในโรงพยาบาล (Thai HADS) วิเคราะห์ข้อมูลโดยใช้สถิติบรรยาย และสัมประสิทธิ์ สหสัมพันธ์เพียร์สัน ผลการวิจัยพบว่า กลุ่มตัวอย่างร้อยละ 48.3 มีความวิตกกังวลในระดับต่ำ ร้อยละ 33.3 ระดับปานกลาง และร้อยละ 18.3 ระดับรุนแรงด้านภาวะซึมเศร้าร้อยละ 70 อยู่ในระดับต่ำ ร้อยละ 20 อยู่ในระดับปานกลาง และร้อยละ 10 อยู่ในระดับรุนแรง ส่วนปัจจัยส่วนบุคคลไม่มีความสัมพันธ์กับความวิตกกังวลและภาวะซึมเศร้า จากผลการศึกษาแสดงให้เห็นว่า หญิงตั้งครรภ์ควรได้รับการคัดกรองความวิตกกังวลและภาวะซึมเศร้าที่หน่วยฝากครรภ์ เพื่อให้การดูแลก่อนเกิดภาวะเจ็บครรภ์ก่อนกำหนดได้อย่างถูกต้องเหมาะสม

This study aimed at investigating: 1) the rate and level of anxiety and depression in pregnant women with preterm labor and 2) the relationship between the selected factors, included ages, education levels, occupations, family incomes with anxiety and depression in pregnant women with preterm labor. The research participants consisted of 60 pregnant women with preterm labor were recruited between 24 - 36 weeks of gestation in Ramathibodi Hospital from September 2013 to March 2014 by purposive sampling. Participants completed the Demographic Questionnaire and Thai HADS Questionnaire (to assess anxiety and depression ) within 24 hours after admission. The data were analyzed using descriptive statistics and Pearson’s product moment correlation. The results revealed that the participants 48.3% had mild anxiety scores, 33.3% had moderate anxiety scores, and 18.3% had severe anxiety scores, but the depression scores 70% were mild, 30% moderate, and 10% severe. The selected factors were not correlated with anxiety and depression. This study suggests that nurses should have a screening and assessment of anxiety and depression among pregnant women at antenatal clinic to provide proper management before preterm labor pain and develop the guideline for effective nursing intervention in this group.
