ปัจจัยทํานายระยะเวลาการฟื้นตัวหลังผ่าตัดในห้องพักฟื้นในผู้ป่วยผ่าตัดมดลูกทางหน้าท้อง

Abstract

วัตถุประสงค์: เพื่อศึกษาอํานาจการทํานายของการได้รับข้อมูลก่อนผ่าตัด ชนิดการระงับความรู้สึก ระยะเวลาผ่าตัดและปริมาณเลือดที่สูญเสียระหว่างผ่าตัด ต่อระยะเวลาการฟื้นตัวหลังผ่าตัดในห้องพักฟื้นในผู้ป่วยผ่าตัดมดลูกทางหน้าท้องรูปแบบการวิจัย: การศึกษาเชิงทํานาย (Predictive study) วิธีดําเนินการวิจัย: กลุ่มตัวอย่างคือ ผู้ป่วยสตรีอายุตั้งแต่ 18 ปีขึ้นไป มารับการผ่าตัดมดลูกทางหน้าท้องตามนัด แผนกนรีเวชวิทยา โรงพยาบาลศิริราช จํานวน 88 คน เก็บรวบรวมข้อมูลโดยใช้แบบบันทึกข้อมูลและแบบสอบถาม วิเคราะห์ข้อมูลโดยใช้สถิติเชิงบรรยาย และสถิติถดถอยพหุคูณแบบเป็นขั้นตอน ผลการวิจัย: พบว่าระยะเวลาการฟื้นตัวหลังผ่าตัดในห้องพักฟื้นเฉลี่ย 99.3 นาที (SD = 14.6) โดยมีระยะการฟื้นตัวหลังผ่าตัดในห้องพักฟื้นจนพร้อมจําหน่ายที่เวลา 90 และ 105 นาที (ร้อยละ 39.8 และ 31.1) ตามลําดับ ระยะเวลาการฟื้นตัวหลังผ่าตัดในห้องพักฟื้นที่นานที่สุดคือ 135 นาที (ร้อยละ 3.4) และพบว่าระยะเวลาผ่าตัดและการระงับความรู้สึกทั่วร่างกาย สามารถร่วมกันทํานายระยะเวลาการฟื้นตัวหลังผ่าตัดในห้องพักฟื้นในผู้ป่วยผ่าตัดมดลูกทางหน้าท้องได้ร้อยละ 44.4 อย่างมีนัยสําคัญทางสถิติ (R2 = .444, p < .05) สรุปและข้อเสนอแนะ: พยาบาลห้องพักฟื้นควรพัฒนาโปรแกรมหรือกิจกรรมเพื่อส่งเสริมการฟื้นตัวหลังผ่าตัดในห้องพักฟื้น ในผู้ป่วยผ่าตัดมดลูกทางหน้าท้องที่มีระยะเวลาผ่าตัดนานและได้รับการระงับความรู้สึกแบบทั่วร่างกาย

Purpose: To investigate the predictive power of preoperative information received, type of anesthesia, operation time, and estimated blood loss on recovery duration at a post-anesthetic care unitin patients undergoing abdominal hysterectomy.Design: Predictive study.Methods: The participants were 88 female patients aged 18 years old or older who underwent anappointed abdominal hysterectomy at the gynecology clinic, Siriraj Hospital. Data were collected byrecord form and a questionnaire. Descriptive statistics and Stepwise Multiple Regression were used fordata analysis.Main findings: The findings revealed that the mean recovery duration at the post-anesthetic careunit was 99.3 minutes (SD = 14.6). For the recovery durations at the post-anesthetic care unit fromadmission until discharge, 39.8% and 31.1% of the study sample spent 90 and 105 minutes respectively.The maximum time needed for recovery was 135 minutes (3.4% of the sample). It was found thatoperation time and general anesthesia could jointly predict recovery duration at post-anesthetic careunit in patients undergoing abdominal hysterectomy by 44.4% (R2 = .444, p < .05).Conclusion and recommendations: The findings suggest that perioperative nurses shoulddevelop interventions or programs to promote post-anesthetic recovery in abdominal hysterectomypatients, particularly for those who had a long operation time and have received general anesthesia
