ความสัมพันธ์ระหว่างการรับรู้สิ่งก่อความเครียดและอาการซึมเศร้าในวัยรุ่นโรคธาลัสซีเมีย

Abstract

วัตถุประสงค์:เพื่อศึกษาความสัมพันธ์ระหว่างการรับรู้สิ่งก่อความเครียดและอาการซึมเศร้าของวัยรุ่นโรคธาลัสซีเมีย รูปแบบการวิจัย: การวิจัยเชิงพรรณนาวิเคราะห์ความสัมพันธ์ วิธีดําเนินการวิจัย: กลุ่มตัวอย่างเป็นวัยรุ่นโรคธาลัสซีเมียอายุ 10-16 ปี จํานวน 100 ราย มารับการตรวจรักษาในคลินิกโรคเลือด ของโรงพยาบาลแห่งหนึ่งในจังหวัดเชียงราย เลือกกลุ่มตัวอย่างแบบสะดวกตามเกณฑ์การคัดเข้าและคัดออกเก็บข้อมูลโดยใช้แบบบันทึกข้อมูลส่วนบุคคล แบบสัมภาษณ์การรับรู้สิ่งก่อความเครียดของวัยรุ่นโรคธาลัสซีเมีย และแบบคัดกรองภาวะซึมเศร้าในเด็ก วิเคราะห์ข้อมูลด้วยสถิติพรรณนาและสัมประสิทธิ์สหสัมพันธ์เพียร์สันผลการวิจัย:กลุ่มตัวอย่างร้อยละ 28 มีอาการซึมเศร้า โดยมีคะแนนเฉลี่ยของอาการซึมเศร้าเท่ากับ 17.54 (SD = 1.92) มีการรับรู้การมีสถานการณ์สิ่งก่อความเครียดเกิดขึ้น ตั้งแต่ 11 ถึง 47 สถานการณ์ (X = 30.39, SD = 7.22) มีคะแนนการรับรู้ความรุนแรงของสถานการณ์ระหว่าง 1 ถึง 34 คะแนน หรือเฉลี่ย 15.55 (SD = 10.19) และการรับรู้สิ่งก่อความเครียดมีความสัมพันธ์ทางบวกกับอาการซึมเศร้า (r = .70, p < 0.01) สรุปและข้อเสนอแนะ: พยาบาลและทีมสุขภาพควรคัดกรองอาการซึมเศร้าในวัยรุ่นโรคธาลัสซีเมีย และควรจัดกิจกรรมการพยาบาลเพื่อส่งเสริมการเผชิญความเครียด และการปรับตัวที่เหมาะสม

Purpose: To examine the relationship between perceived stressors and depressive symptoms in adolescents with thalassemia.Design: Descriptive correlational study.Methods: The sample consisted of 100, 10- to 16-year-old adolescents with thalassemia who cameto receive treatment at the Hematological Clinic of a hospital in Chiangrai Province. The sample wasselected by convenience sampling according to the inclusion and exclusion criteria. Data were collectedusing demographic data record forms, interviewing questionnaires on perceived stressors of adolescentswith thalassemia and the Children’s Depression Inventory. Data analysis was carried out withdescriptive statistics and Pearson’s product moment correlation coefficients.Main findings: The research findings revealed that 28% of the sample have depressive symptomswith mean scores of 17.54 (SD = 1.92). The subjects reported that 11 to 47 stressful situations (X = 30.39,SD = 7.22) did happen and average severity score of perceived stressors was 15.55 (SD = 10.19), rangingfrom 1-34. Perceived stressors were found to be positively related to depressive symptoms (r = .70,p < 0.01).Conclusion and recommendations: Nurses and healthcare teams should screen for depressivesymptoms in adolescents with thalassemia. Nursing intervention should be developed to help theadolescents have proper stress coping and adjustment.
