ประสิทธิผลของโปรแกรมการดูแลต่อเนื่องในผู้ป่วยหลังผ่าตัดลิ้นหัวใจที่ได้รับยาวาร์ฟารินต่อความสม่ำเสมอในการรับประทานยาวาร์ฟารินและพฤติกรรมการบริโภคอาหาร

Abstract

วัตถุประสงค์: เพื่อศึกษาประสิทธิผลของโปรแกรมการดูแลต่อเนื่องในผู้ป่วยหลังผ่าตัดลิ้นหัวใจที่ได้รับยาวาร์ฟารินต่อความสม่ำเสมอในการรับประทานยาวาร์ฟารินและพฤติกรรมการบริโภคอาหารรูปแบบการวิจัย: การวิจัยเชิงทดลอง วิธีดําเนินกรวิจัย: กลุ่มตัวอย่างคือ ผู้ป่วยหลังผ่าตัดลิ้นหัวใจที่ได้รับยาวาร์ฟารินที่โรงพยาบาลมหาวิทยาลัย จํานวน 68 ราย ดําเนินการตั้งแต่กรกฎาคม 2556 ถึงมีนาคม 2557 กําหนดกลุ่มตัวอย่างโดยการสุ่มตัวอย่างเข้ากลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 34 ราย กลุ่มควบคุมได้รับการดูแลตามปกติ ส่วนกลุ่มทดลองได้รับโปรแกรมการดูแลต่อเนื่องในผู้ป่วยหลังผ่าตัดลิ้นหัวใจที่ได้รับยาวาร์ฟาริน เก็บข้อมูลโดยใช้แบบสอบถามข้อมูลส่วนบุคคล แบบประเมินความเสี่ยงทางด้านร่างกายและการจําหน่ายผู้ป่วยแบบบันทึกความสม่ำเสมอในการรับประทานยาวาร์ฟาริน และแบบบันทึกพฤติกรรมการบริโภคอาหาร วิเคราะห์ข้อมูลโดยใช้สถิติ Mann-Whitney U test, Independent t test และ Pair t-test ผลการวิจัย: กลุ่มทดลองมีความสม่ำเสมอในการรับประทานยาวาร์ฟารินในสัปดาห์ที่ 12 สูงกว่ากลุ่มควบคุมอย่างมีนัยสําคัญทางสถิติ (p = .00) และมีพฤติกรรมการบริโภคอาหารในสัปดาห์ที่ 4 และ 12 สูงกว่ากลุ่มควบคุมอย่างมีนัยสําคัญทางสถิติ (p = .00, p = .00) สรุปและข้อเสนอแนะ: ทีมพยาบาลผู้ดูแลควรพัฒนาโปรแกรมดังกล่าวอย่างต่อเนื่องในการดูแลผู้ป่วยหลังผ่าตัดลิ้นหัวใจที่ได้รับยาวาร์ฟาริน เพื่อส่งเสริมให้ผู้ป่วยมีความสม่ำเสมอในการรับประทานยาวาร์ฟาริน และมีพฤติกรรมการบริโภคอาหารอย่างเหมาะสมมากขึ้น

Purpose: This experimental research study was conducted to determine the effectiveness of continuing care program for patients after valvular heart surgery receiving warfarin on warfarin adherence and dietary consumption behavior. Design: Experimental research design. Methods: The samples consisted of 68 patients with valvular heart disease who underwent a valvular heart surgery at a University Hospital and were receiving warfarin during the period from July 2013 to March 2014. The sample were randomly assigned into a control group and an experimental group, each group had 34 patients. The control group received usual care while the experimental group received the continuing care program for valvular heart surgery patients receiving warfarin. Data were collected by using a questionnaire composing of personal demographics, physical risks and discharge assessment were analyzed by using Chi-square, warfarin adherences scales was analyzed by using a Mann-Whitney U test and dietary consumption behavior was analyzed by using independent t- test and paired t-test.Main findings: For the experimental group, the warfarin adherence score at the 12th week was significantly higher than that of the control group (p = .00). The dietary consumption behavior scores at the 4thand 12th week were significantly higher than those of the control group (p = .00, p = .00).Conclusion and recommendations: Nurses and the multidisciplinary care team should develop a continuing care program for valvular heart surgery patients receiving warfarin in order to ensure the warfarin adherence and dietary consumption behavior.
