A 36-year-old woman with papillary thyroid carcinoma underwent total thyroidectomy and received 3 separate 150 mCi doses of I-131 about 6 months apart. A scan 3 days after the first ablative dose and second dose for high serum thyroglobulin (Tg) of 48.1 ng/ml revealed focal uptake in bilateral pelvic regions, corresponding to the bilateral adnexal masses on SPECT-CT. Bilateral ovarian cystectomy was performed and histopathology revealed bilateral ovarian endometrial cysts. Two months after surgery, a scan 3 days following the third dose for persistently high serum Tg of 20 ng/mL was negative. The level of serum Tg dropped to 1ng/mL 6 months later.
