Abstract

อาการกรนที่เกิดขึ้นในขณะหลับ เป็นเสียงที่เกิดจากการสั่นสะเทือนภายในช่องทาง เดินหายใจที่ตีบแคบลงจากหลายสาเหตุ เสียงกรนที่ดังมากขึ้นเกิดจากช่องทางเดินหายใจตีบ แคบมากจนอากาศไม่สามารถผ่านเข้า-ออก ได้อย่างสะดวก จะทำให้การหายใจติดขัดจนถึงขั้น อุดตันและเสียชีวิตได้ เนื่องจากสมองทำหน้าที่ควบคุมระบบหายใจเมื่อการหายใจและการแลก เปลี่ยนก๊าซไม่มีประสิทธิภาพ เกิดภาวะพร่องออกซิเจนในเลือด สมองจึงต้องตื่นตัวเป็นระยะใน ขณะหลับเพื่อสั่งให้ระบบการหายใจทำงาน ส่งผลให้สมรรถภาพในการนอนหลับลดลง ถ้าเป็น อยู่อย่างต่อเนื่องนานนับปีการนอนหลับที่ไม่มีคุณภาพจะเริ่มทำให้การทำงานในช่วงกลางวัน ไม่มีประสิทธิภาพเกิดอาการง่วงขณะทำงาน อาจมีหลับในและเกิดอันตรายขณะปฏิบัติงาน ระบบการหายใจไม่มีประสิทธิภาพเกิดภาวะพร่องออกซิเจนในเลือดอาจส่งผลให้สมองขาด ออกซิเจน หรือกล้ามเนื้อหัวใจโตจากการทำงานหนัก การตรวจการนอนหลับจึงจำเป็นสำหรับ ผู้ที่นอนกรนมานานแบบเรื้อรัง เพื่อประเมินความผิดปกติและความรุนแรงของระบบสมองและ ระบบหายใจ ซึ่งการตรวจการนอนหลับโดยเจ้าหน้าที่ที่มีความชำนาญ และแพทย์ที่มีความ เชี่ยวชาญ ผู้ป่วยจะได้รับการรักษาที่ถูกต้องเหมาะสม สามารถแก้ไขให้มีการนอนหลับและระบบ หายใจทำงานเป็นปกติ มีประสิทธิภาพ คุณภาพในการดำเนินชีวิตก็จะดีขึ้นเป็นลำดับ

Snoring is the sound that is originated from vibration of upper airway passage and associated with their obstruction. The upper airway obstruction is contributed by the abnormalities of soft tissue and bony structure as well as many contributors. The difficult breathing occurs while sleep especially stage of deep sleep, resulting in repetitive episode of arterial hypoxemia and arousal. These nocturnal phenomena are associated with increase sympathetic activity in terms of increased heart rate and also blood pressure during the whole night. The repeated awakening or poor sleep quality from the depleted oxygenation will insult central nervous system function and development. Longstanding sleep deprivation will cause significant daytime problems for instance loss of vigilance, poor concentration and low level of awareness in routine job (and serious motor vehicle accident.) Sleep study or standard polysomnography is the diagnostic method for document the episodes of sleep disorder breathing and other nocturnal events. The diagnostic test is polychannel record that is required well-trained medical personal so called polysomnographic technician. The data from sleep studies will be beneficial for confirming the diagnosis, assessment of severity of sleep apnea as well as determination of accurate positive pressure level for splinting upper airway. The treatment of upper airway obstruction during sleep by appropriate pressure will eliminate the apnic and hypopnic episodes and provide resuming good sleep quality and patients quality of life.
