A 79-year-old man with diffuse large B-cell lymphoma was presented for assessment of metabolic response to chemotherapy. The baseline F-18 FDG PET showed extensive nodal, soft tissue, and bone metastases, which have completely resolved on the subsequent follow-up study, except for persistent perianal FDG activity. The perianal FDG activity was explained by inflammatory changes, secondary to hemorrhoids evident on the corresponding contrast-enhanced CT study. This exemplified the application of FDG PET/CT in detecting benign inflammatory disorders and the importance of correlation with CT to minimize misdiagnosis of residual malignancy.
