ผลของการนวดร่วมกับการประคบร้อนต่อความเจ็บปวดและการเผชิญความเจ็บปวดของผู้คลอดครรภ์แรก

Abstract

วัตถุประสงค์: เพื่อศึกษาผลของการนวดร่วมกับการประคบร้อนต่อความเจ็บปวดและการเผชิญความเจ็บปวดของผู้คลอดครรภ์แรกในระยะที่ 1 ของการคลอด รูปแบบการวิจัย:การวิจัยกึ่งทดลอง วิธีดําเนินการวิจัย: กลุ่มตัวอย่างเป็นผู้คลอดครรภ์แรกจํานวน 70 ราย ที่มาคลอดบุตรในห้องคลอด โรงพยาบาลบางจาก คัดเลือกกลุ่มตัวอย่างตามเกณฑ์ที่กําหนด แบ่งเป็นกลุ่มทดลอง 35 รายที่ได้รับการนวดร่วมกับการประคบร้อนและการพยาบาลตามปกติ และกลุ่มควบคุม 35 รายที่ได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบแผนการนวดร่วมกับการประคบร้อนด้วยลูกประคบข้าวสารเหนียวดํา แบบสัมภาษณ์ข้อมูลส่วนบุคคล แบบประเมินความเจ็บปวดด้วยสายตา แบบสังเกตพฤติกรรมการเผชิญความเจ็บปวด วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติทดสอบค่าที และสถิติการวิเคราะห์ความแปรปรวนร่วม ผลการวิจัย: ผลการศึกษาพบว่า กลุ่มที่ได้รับการนวดร่วมกับการประคบร้อนในระยะที่ 1 ของการคลอดมีค่าเฉลี่ยของคะแนนความเจ็บปวดน้อยกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสําคัญทางสถิติ (p < .001) และมีค่าเฉลี่ยของคะแนนพฤติกรรมการเผชิญความเจ็บปวดสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสําคัญทางสถิติ (p < .001) สรุปและข้อเสนอแนะ: การนวดร่วมกับการประคบร้อนด้วยลูกประคบข้าวสารเหนียวดําสามารถ ลดการเจ็บปวดครรภ์ได้ พยาบาลผดุงครรภ์จึงสามารถนําวิธีการดังกล่าวไปใช้กับผู้คลอดครรภ์แรก ในการลดความเจ็บปวดและเผชิญกับความเจ็บปวดได้อย่างเหมาะสมในระยะคลอด

Purpose: The present study aimed to investigate the effects of massage and hot compress on laborpain and pain coping behaviors of primigravidas in the first stage of labor.Design: Quasi–experimental design.Methods: The subjects of this study were pregnant women who were in labor at the Bangchakhospital. Seventy primigravidas were chosen according to inclusion criteria. The samples were dividedinto an experimental and a control group with 35 subjects in each group. The experimental groupparticipated in the massage and hot compress protocol. The control group received usual nursing care.The research instruments were composed of a massage and hot compress protocol, patient’s interviewform, visual analougue scale, and labor pain coping scale. The data were analyzed using descriptivestatistics, independent t-test, and analysis of covariance.Main findings: The findings revealed that the subjects who participated in the massage and hotcompress protocol in the first stage of labor had lower mean pain score than those in the control group.(p < .001) and had greater mean labor pain coping score than those in the control group (p < .001).Conclusion and recommendations: The massage and hot compress protocol can reducing laborpain, so nurse midwives should offer massage and hot compress as a nursing intervention forprimigravidas in reducing their labor pain and enhancing appropriate pain coping behavior in labor.
