พฤติกรรมของผู้ดูแลในการดูแลเด็กป่วยด้วยโรคเรื้อรังอายุต่ำกว่า 5 ปี

Abstract

การวิจัยเชิงสำรวจนี้ มีวัตถุประสงค์เพื่อศึกษาพฤติกรรมของผู้ดูแลในการให้การดูแลเด็กป่วยด้วยโรค เรื้อรังที่มีอายุต่ำกว่า 5 ปี และปัจจัยที่กำหนดของผู้ดูแลเด็ก กลุ่มตัวอย่าง คือ ผู้ดูแลเด็กป่วยด้วยโรคเรื้อรังที่มีอายุ ต่ำกว่า 5 ปี ที่มารับการรักษาที่โรงพยาบาลรามาธิบดี จำนวน 176 คน เครื่องมือที่ใช้ในการวิจัย คือ แบบสัมภาษณ์ ซึ่งผ่าน การพัฒนาและตรวจสอบความตรงตามเนื้อหาและความเชื่อมั่นในระดับที่ยอมรับได้ เก็บรวบรวบข้อมูลโดย การสัมภาษณ์ตัวอย่างเป็นรายบุคคล วิเคราะห์ข้อมูลด้วยสถิติร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และ วิเคราะห์ความสัมพันธ์ด้วย สถิติไคสแควร์ และ ANOVA ผลการศึกษา พบว่า พฤติกรรมการดูแลเด็กป่วยโรคเรื้อรังที่มีอายุต่ำกว่า 5 ปีในเรื่องการติดตามการ รักษา การให้ยา การฟื้นฟูสมรรถภาพ/กระตุ้นพัฒนาการ และการดูแลด้านจิตใจ โดยภาพรวมผู้ดูแลส่วนใหญ่มี ระดับการดูแลอยู่ในระดับพอใช้ร้อยละ 74.4 รองลงมาอยู่ในระดับดีร้อยละ 22.2 และระดับไม่ดีร้อยละ 3.4 ส่วน ผลกระทบต่อผู้ดูแลในการให้การดูแลเด็กป่วยโรคเรื้อรังที่มีอายุต่ำกว่า 5 ปี นั้น มี 4 ด้านได้แก่ ด้านการงาน การเงิน ความเป็นส่วนตัวและด้านจิตใจ โดยภาพรวมแล้วผู้ดูแลได้รับผลกระทบอยู่ในระดับปานกลาง สูง และต่ำ ร้อยละ 59.1, 31.8 และ 9.1 ตามลำดับ นอกจากนี้ยังพบว่า ปัจจัยด้านอายุ ระดับการศึกษา สถานภาพสมรส อาชีพ สถานะ ทางเศรษฐกิจของครอบครัว การรับรู้ความรุนแรงของโรคและการได้รับแรงสนับสนุนจากสังคมมีความสัมพันธ์อย่าง มีนัยสำคัญทางสถิติ กับพฤติกรรมการให้การดูแลเด็ก (p<0.05) ซึ่งผลการวิจัยจะเป็นประโยชน์ต่อโรงพยาบาลและ หน่วยงานที่เกี่ยวข้องในการวางแผนการจัดบริการส่งเสริมการสร้างพฤติกรรมการดูแลที่มีคุณภาพ ให้แก่ผู้ดูแลเด็ก และการปรับปรุงสถานบริการได้ครบวงจรด้วย

This survey research aimed at studying caregiver’s care behavior in children with chronic illness under 5 years and the factors that determined the behavior. The sample was 176 caregivers who provided care for chronically ill children under 5 years who were hospitalized at Ramathibodi Hospital. The instrument used for data collection was the interview-schedule that was developed and tested for content validity and reliability at an acceptable level. Data were collected by interviewing each member of the sample individually. Data analysis was done by computing percentages, means, standard deviations, and relationship analysis was done by using the Chi-Square test and ANOVA. The results showed that in general, most of the caregivers performed a “moderate” level of care behavior for these children in regard to treatment follow-up, medication, rehabilitation/stimulating development, and mental health care (77.4%), while 22.2 percent, and 3.4 percent performed at “good” and “not good” levels, respectively. Four aspects of the impacts of performing care behavior for children with chronic illness under 5 years on the caregivers were found as follows: work; finance; privacy; and mental health. The impacts were found to be at “moderate”, “high” and “low” levels (59.1%, 31.8%, and 9.1%, respectively). Besides, it was found that the factors with regard to age, educational level, marital status, occupation, family’s economic status, perceived severity of disease, and receiving social support related significantly with the quality of care behavior (p<0.05). The findings of this study can be beneficial for hospitals and related organizations who are responsible for promoting quality care behavior of caregivers of children with chronic illness, as well as for improving health service organizations to be come comprehensive.
