อิทธิพลของความวิตกกังวลของผู้ปกครองต่อระดับความกลัว/ความวิตกกังวลในการทำฟันในเด็กไทยกลุ่มหนึ่ง

Abstract

วัตถุประสงค์: เพื่อหาระหว่างความสัมพันธ์ระหว่างความกลัวและความวิตกกังวลของผู้ปกครองและของเด็กต่อการเข้ารับการรักษาทางทันตกรรม รวมทั้งบัจจัยของผู้ปกครองที่มีอิทธิพลต่อความกลัวของเด็ก วัสดุอุปกรณ์และวิธีการศึกษา: เป็นการวิจัยแบบตัดขวางโดยใช้แบบสอบถามและสัมภาษณ์ผู้ปกครองและเด็กอายุระหว่าง 7 ถึง 12 ปี โดยตรง ที่คลินิกทันตกรรมเด็ก คณะทันตแพทยศาสตร์ มหาวิทยาลัยมหิดล แบบสอบถามมี 2 ชุด สำหรับใช้กับผู้ปกครองและสำหรับเด็ก เก็บข้อมูลของผู้ปกครอง เพศความสัมพันธ์กับผู้ป่วยเด็ก ระดับการศึกษา อาชีพ รายได้ต่อเดือน และประวัติการรักษาทางทันตกรรม ได้แก่ ความถี่ ประสบการณ์ด้านลบที่ได้รับจากการทางทันตกรรม เก็บข้อมูลของเด็ก อายุ เพศ จำนวนพี่น้อง ลำดับบุตร และประวัติการรักษาทางทันตกรรมได้แก่ จำนวนครั้งที่เข้ารับการรักษาทางทันตกรรมประเภทของการรักษาที่เคยได้รับ ประสบการณ์ด้านลบจากการรับการรักษาทางทันตกรรม โดยทั้งผู้ปกครองและเด็กใช้โมดิฟายหว่องเบเกอร์เฟสเพนสเกลวิเคราะห์ข้อมูลโดยใช้สถิติไคร์สแควร์และอโนว่า ผลการศึกษา: กลุ่มตัวอย่างมีทั้งหมดจำนวน 93 คู่ ผู้ปกครองส่วนใหญ่เลี้ยงดูเด็กเอง ร้อยละ 48.4 เคยมีประสบการณ์ไม่ดีในการรักษาทางทันตกรรมสำหรับเด็กนั้นร้อยละ 69.9 ไม่เคยมีประสบการณ์ไม่ดีในการเข้ารับการรักษาทางทันตกรรม ค่าเฉลี่ยระดับความกลัวและความวิตกกังวลของผู้ปกครองและของเด็กไม่มีความแตกต่างกันอย่างมีนัยสำคัญ รวมทั้งระดับความกลัวและความวิตกกังวลของผู้ปกครองมีความสัมพันธ์กับระดับความกลัวและความวิตกกังวลของเด็กอย่างมีนัยสำคัญ (ค่าสหสัมพันธ์ = 0 ระดับนัยสำคัญทางสถิติที่ 0.01) นอกจากนี้ ยังพบว่าประสบการณ์ไม่ดีที่เด็กเคยได้รับนำไปสู่ความกลัวและวิตกกังวลในการเข้ารับการรักษาครั้งต่อไป การอบรม เลี้ยงดู และการเล่าประสบการณ์ที่ไม่ดีให้ฟังไม่ส่งผลต่อระดับความกลัวและความวิตกกังวลของเด็กในช่วงอายุนี้ บทสรุป: ระดับความวิตกกังวลและความกลัวของผู้ปกครองสัมพันธ์กับระดับความวิตกวังวลและความกลัวของเด็ก

Objectives: To investigate the relationship between parents and their childûs fear and anxiety levels during receiving dental treatments and to examine the parental influence over childûs dental fear. Materials and Methods: This is a cross-sectional clinical observation by mean of questionnaires and direct interview. There were two sets of the questionnaire; one for parents and the other for their child. Parent general data; sex, age, education levels, monthly income, and the unsatisfied experiences during receiving dental treatments in the past were collected. Children general data ; sex, age, birth order, number of siblings the unsatisfied experiences during receiving dental treatments in the past, ineluding type of received procedures, and frequency of dental visit were also collected. Both of them used the Modified Wong Baker face pain scale. Data was analyzed by Chi-square statistics and ANOVA. Results: Total of ninety-three pairs of parent and their child (aged between 7-12 yr.) were participated. Forty eight percents of parents encountered bad dental experiences in the past. Seventy percents of child participants never had bad dental experiences. An average of parentûs fear and anxiety levels was not significantly different from their childûs. Parent fear and anxiety levels were significantly associated with those of their child (r=0 at p<0.001). In children, gaining bad experiences from previous dental treatment led to the development of fear and anxiety in the next appointment. Parenting styles and telling their childûs bad dental experiences were not associated with childûs fear and anxiety levels in these age group. Conclusion: Parental fear and anxiety levels during receiving dental treatments were significantly associated with those of their child.
