DCollective: A Configurable Electronic Data Collection Form and Information Dashboard

© 2018 IEEE. Collecting effective healthcare survey data requires a novel approach differed from traditional paper-based methods where manual electronic data entry would be required. Inaccuracy and mistakes can occur at any points from gathering, transporting to entering collected data into a computer. dCollective is a cloudbased software application designed to assist healthcare workers to design customized online survey forms for interviewers, patients and caretakers. The workers create, manage and assign electronic forms to their target groups. Survey forms can be logically stored and retrieved based on various variables such as patient's age, gender, medical state and clinical status. In addition to typical data gathering from paper forms, a dCollective form may include geolocation along with picture, audio and video. Data from each form is extracted to construct longitudinal information pertaining to each patient. This information will be used alongside respected patient's medical record to create visual representations and statistical reports for each patient or a group of patients based on a set of search criteria.
