ผลของโปรแกรมการดูแลผู้ป่วยเบาหวานที่มีปัญหาซับซ้อนโดยผู้ปฏิบัติการพยาบาลขั้นสูงต่อผลลัพธ์ด้านผู้ป่วย

Abstract

การวิจัยนี้เป็นการวิจัยกึ่งทดลองศึกษาแบบกลุ่มเดียววัดซ้ำ มีวัตถุประสงค์เพื่อศึกษา ผลของโปรแกรมการดูแลผู้ป่วยเบาหวานที่มีปัญหาซับซ้อนโดยผู้ปฏิบัติการพยาบาลขั้นสูง (APN) ต่อผลลัพธ์ด้านผู้ป่วยกรอบแนวคิดครั้งนี้ใช้ทฤษฎีการดูแลตนเองของโอเร็มร่วมกับการใช้เทคนิค การสัมภาษณ์เพื่อสร้างแรงจูงใจในการปรับเปลี่ยนพฤติกรรมและหลักฐานเชิงประจักษ์ผู้เข้าร่วมวิจัย เป็นผู้ป่วยเบาหวานที่มีปัญหาซับซ้อนเข้ารักษาในโรงพยาบาลรามาธิบดี จำนวน 30 ราย เลือกกลุ่ม ตัวอย่างแบบเฉพาะเจาะจงตามเกณฑ์คัดเข้าผู้เข้าร่วมวิจัยได้รับโปรแกรม 3 ระยะ เป็นเวลา 10 สัปดาห์ ในสัปดาห์ที่ 1 มีกิจกรรมระยะที่ 1 และระยะที่ 2 ดังนี้ ระยะที่ 1 ส่งเสริมการรู้จักตนเองและสร้างแรงจูงใจ โดย APN ให้ผู้เข้าร่วมวิจัยประเมินภาวะสุขภาพกระตุ้นสร้างแรงจูงใจในการดูแลตนเองระยะที่ 2 ตั้งเป้าหมายและวางแผนร่วมกันให้ความรู้ในการดูแลเบาหวานฝึกทักษะและพัฒนาทักษะในการดูแล ตนเองสร้างสิ่งแวดล้อม และสร้างแรงจูงใจรวม จำนวน 3 ครั้งและในสัปดาห์ที่ 2 ดำเนินกิจกรรมระยะที่ 3 สนับสนุนการดูแลต่อเนื่องโดยติดตามทางโทรศัพท์ทุก 2 สัปดาห์ รวมทั้งสิ้น 5 ครั้งและเปิดช่องทาง สายด่วน APN 24 ชั่วโมง เก็บข้อมูลพฤติกรรมการดูแลตนเองคุณภาพชีวิตและค่าน้ำตาลเฉลี่ยสะสม ในเลือดก่อนและหลังสิ้นสุดโปรแกรม 2 สัปดาห์ เก็บข้อมูลความรู้เกี่ยวกับเบาหวาน 3 ครั้ง คือ ก่อน และหลังให้ความรู้ทุกด้านและหลังสิ้นสุดโปรแกรม 2 สัปดาห์ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงบรรยาย การทดสอบทีและสถิติความแปรปรวนแบบวัดซ้ำของฟรีดแมนผลการวิจัย พบว่า ผู้เข้าร่วมวิจัยมีอายุ ระหว่าง 19 ถึง 87 ปี มีค่าเฉลี่ยคะแนนความรู้พฤติกรรมการดูแลตนเอง และคุณภาพชีวิตสูงกว่าก่อน ได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติและมีค่าเฉลี่ยน้ำตาลสะสมเฉลี่ยลดลงจาก 11.43% เป็น 7.29% หลังสิ้นสุดโปรแกรมงานวิจัยนี้แสดงถึงประสิทธิภาพของโปรแกรมการดูแลผู้ป่วยเบาหวาน ที่มีปัญหาซับซ้อนโดยผู้ปฏิบัติการพยาบาลขั้นสูงอย่างต่อเนื่องต่อผลลัพธ์ที่ดีด้านผู้ป่วยดังนั้นจึงควร นำโปรแกรมนี้ลงสู่การปฏิบัติแก่ผู้ป่วยเบาหวานที่ไม่สามารถคุมระดับน้ำตาลในเลือดได้และมีปัญหา ซับซ้อนให้มีคุณภาพชีวิตที่ดี

The purpose of this one group pre-post quasi experimental study was to examine the effect of advanced practice nurse (APN)- led intervention program on knowledge, self-care behaviour, quality of life, and glycemic control in diabetes patients with complex problems. Orem’s Self- Care Theory combined with the motivational interviewing, and evidence-based approach were used as the theoretical framework of this study. A total of 30 diabetes patients admitted to Ramathibodi Hospital was recruited by purposive sampling. The program consisted of 3 phases within 10 weeks. At the first week, the APN visited patients for 3 times and conducted the program in Phase I and II. The program included 1) Phase I: promoting self-awareness of patient health status and raising motivation by the APN, and 2) Phase II: planning and goal setting for self-care modification, developing self-care abilities via individual education and skill training and facilitate appropriately resource mobilization for self-monitoring. In the second week, Phase III including empowerment to continue self-care by the APN using home telephone counselling in every 2 weeks for 5 times was done. In case of emergency, the APN opened hotline telephone call for 24 hours. Self-care behaviour, quality of life, and glycemic control were measured at baseline and 2 weeks after completion of the program. Knowledge was measured 3 times at baseline, after finishing education training, and 2 weeks after completion of the program. Descriptive statistics, paired t-test and Friedman test were used to analyse the data. The study findings showed that the mean scores of knowledge, self-care behaviour and quality of life were significantly higher than those before entering the program, and the mean glycosylated hemoglobin (HbA1c) decreased significantly from 11.43% to 7.29% at posttest. In conclusion, the findings demonstrate the effectiveness of the APN-led intervention program with continuity of care in diabetes patients who have complex problems on their favorable outcomes. Therefore, this program should be implemented in diabetes patients with complex problems to promote their quality of life.
