คุณภาพชีวิตเกี่ยวกับภาวะสุขภาพของผู้ป่วยแผลไหม้

Abstract

การวิจัยเชิงบรรยายครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาคุณภาพชีวิตของผู้ป่วยแผลไหม้ ก่อนและหลังได้รับอุบัติเหตุ ใช้กรอบแนวคิดคุณภาพชีวิตเกี่ยวกับภาวะสุขภาพ โดยเลือกกลุ่ม ตัวอย่างแบบเฉพาะเจาะจง จำนวน 36 คน อายุมากกว่า 15 ปี มีความรุนแรงของแผลไหม้ระดับ ปานกลางถึงรุนแรง ที่โรงพยาบาลรามาธิบดีและโรงพยาบาลนพรัตนราชธานี ผู้วิจัยเก็บรวบรวม ข้อมูลโดยใช้แบบสอบถามและการสัมภาษณ์ เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบบันทึกข้อมูล ส่วนบุคคล แบบประเมินคุณภาพชีวิตเกี่ยวกับภาวะสุขภาพทั่วไป และแบบประเมินคุณภาพชีวิต เกี่ยวกับภาวะสุขภาพแบบเฉพาะโรค วิเคราะห์ข้อมูลโดยใช้สถิติบรรยายและสถิติทดสอบทีข้อมูล เชิงคุณภาพที่ได้จากการตอบแบบสอบถามวิเคราะห์ข้อมูลโดยการวิเคราะห์เนื้อหา ผลการวิจัยพบ ว่า คุณภาพชีวิตเกี่ยวกับภาวะสุขภาพทั่วไปของผู้ป่วยแผลไหม้ในระยะก่อนได้รับอุบัติเหตุโดย รวมและรายด้านอยู่ในระดับดี แต่ระยะหลังได้รับอุบัติเหตุแผลไหม้ พบว่า คุณภาพชีวิตเกี่ยวกับ ภาวะสุขภาพทั่วไปโดยรวมอยู่ในระดับดี สำหรับรายด้าน พบว่า คุณภาพชีวิตด้านความสามารถใน การทำหน้าที่ด้านร่างกาย ความเจ็บปวดทางกาย ความกระฉับกระเฉง บทบาทที่ถูกจำกัดเนื่องจาก ปัญหาด้านร่างกายและปัญหาทางด้านอารมณ์ อยู่ในระดับไม่ดี คุณภาพชีวิตเกี่ยวกับภาวะสุขภาพ ทั่วไปโดยรวมระยะก่อนได้รับอุบัติเหตุสูงกว่าหลังได้รับอุบัติเหตุแผลไหม้อย่างมีนัยสำคัญทางสถิติ อย่างไรก็ตาม จากการศึกษาพบว่าคุณภาพชีวิตเกี่ยวกับภาวะสุขภาพแบบเฉพาะโรคโดยรวม หลังได้รับอุบัติเหตุแผลไหม้อยู่ในระดับดี แต่รายด้านพบว่า คุณภาพชีวิตด้านความสามารถใน การดูแลตนเองเบื้องต้น การทำงานและการประกอบอาชีพ ความไวต่อความร้อน อยู่ในระดับไม่ดี ผลการศึกษาสามารถนำมาใช้เป็นข้อมูลและแนวทางในการวางแผนการพยาบาล การส่งเสริมฟื้นฟู คุณภาพชีวิตด้านสุขภาพได้อย่างเหมาะสมกับสภาวะและข้อจำกัดของผู้ป่วยแผลไหม้ให้สามารถ ดำรงชีวิตได้อย่างมีประสิทธิภาพ

The purpose of this descriptive research was to explore health-related quality of life in burn patients, before and after the burn injury. The health-related quality of life was used as the conceptual framework of this study. A purposive sample was selected consisting of 36 patients, who were over 15 years of age and were diagnosed with moderate to severe burns. Data were obtained by surveying patients at Ramathibodi Hospital and Nopparat Rajathanee Hospital in Bangkok. Instruments used for data collection including the Demographic Data Form, the Short Form-36 Version 2 (SF-36V2) and the Burn Specific Health Scale-Brief (BSHS-B). The data were analyzed using descriptive statistics and content analysis. The main finding of this study showed that the overall generic health-related quality of life (SF-36V2) was good before the burn injury, but poor after the burn injury. Each domain of generic health-related quality of life including physical function, bodily pain, vitality, general health, and role limitation due to physical and emotional problems was poor after burn injury. In addition, the results revealed that the generic health-related quality of life before burn injury was significantly higher than after burn injury. Moreover, this study also found that after burn injury overall diseasespecific health-related quality of life (BSHS-B) was good. However, the domains of BSHS-B including simple ability, work, and heat sensitivity were poor. Based on these findings, nurses could use the data for developing a care plan to improve quality of life in burn patients.
