บทบาทขององค์กรปกครองส่วนท้องถิ่นในการสนับสนุนการมารับการรักษาต่อเนื่องในผู้สูงอายุโรคเรื้อรัง: กรณีศึกษา

Abstract

การศึกษาครั้งนี้มีวัตถุประสงค์ เพื่อศึกษาบทบาทขององค์กรปกครองส่วนท้องถิ่นและบทบาท ตามความคาดหวังของประชาชนที่มีต่อองค์กรปกครองส่วนท้องถิ่นในการสนับสนุนการมารับการรักษา ต่อเนื่องของผู้สูงอายุโรคเรื้อรังในเขตเทศบาลตำบลบางกระสั้น อำเภอบางปะอิน จังหวัดพระนครศรีอยุธยา กลุ่มตัวอย่างคือ ผู้สูงอายุโรคเรื้อรังที่อยู่บ้านลำพังเมื่อผู้ดูแลต้องออกไปทำงานนอกบ้าน จำนวน 10 ราย ผู้ดูแลผู้สูงอายุโรคเรื้อรัง จำนวน 10 ราย ผู้บริหารองค์กรปกครองส่วนท้องถิ่น จำนวน 2 ราย และผู้บริหาร สาธารณสุข จำนวน 3 ราย เก็บรวบรวมข้อมูลจากแบบสอบถาม การสนทนากลุ่ม และการสัมภาษณ์ วิเคราะห์ข้อมูลส่วนบุคคลโดยใช้สถิติเชิงบรรยาย ข้อมูลการสนทนากลุ่มและการสัมภาษณ์โดยการวิเคราะห์ เชิงเนื้อหา ผลการศึกษาพบว่า กลุ่มตัวอย่างทุกกลุ่มรับรู้ปัญหาสุขภาพ และผลกระทบที่เกิดจากการเจ็บป่วย ของผู้สูงอายุโรคเรื้อรัง ผู้สูงอายุโรคเรื้อรังแก้ปัญหาสุขภาพโดยเข้ารับการรักษาต่อเนื่อง แต่ยังพบปัญหาว่า ไม่สามารถไปรับการรักษาต่อเนื่องตามนัดได้ทุกครั้ง เพราะผู้ดูแลต้องออกไปทำงานนอกบ้าน และผู้สูงอายุ โรคเรื้อรังมีข้อจำกัดในเรื่องการเดินทางด้วยรถประจำทางสาธารณะโดยลำพัง สำหรับองค์กรปกครอง ส่วนท้องถิ่นพบว่าในปัจจุบันยังไม่มีนโยบายและแผนเกี่ยวกับการดูแลผู้สูงอายุโรคเรื้อรังโดยเฉพาะ จึงยัง ไม่มีบทบาทชัดเจนในการสนับสนุนการมารับการรักษาต่อเนื่องของผู้สูงอายุโรคเรื้อรัง แต่ดำเนินนโยบาย ให้การช่วยเหลือทางด้านสวัสดิการสังคมกับผู้สูงอายุและผู้ยากไร้ มีบริการรถรับส่งกรณีเกิดเจ็บป่วยฉุกเฉิน แต่ไม่ครอบคลุมการไปตรวจตามนัดปกติในภาคสาธารณสุข มีการจัดระบบบริการที่เอื้อและสะดวกต่อ การมารับการรักษาต่อเนื่อง ความคาดหวังของประชาชนที่มีต่อบทบาทขององค์กรปกครองส่วนท้องถิ่น คือ ต้องการได้รับการสนับสนุนบริการรถรับส่งไปตรวจตามแพทย์นัด หากตรงกับวันที่ผู้ดูแลออกไปทำงาน นอกบ้าน และให้มีความร่วมมือกับสาธารณสุขในการพัฒนาศักยภาพสถานีอนามัยให้มีมาตรฐานการดูแล เท่าเทียมกับโรงพยาบาล เพื่อความสะดวกในการเข้าถึงบริการ ผลการศึกษาครั้งนี้มีข้อเสนอแนะคือ ควรมี การจัดตั้งคณะกรรมการในการดำเนินงานให้ผู้สูงอายุโรคเรื้อรังได้รับการรักษาต่อเนื่องโดย 1) องค์กร ปกครองส่วนท้องถิ่นสนับสนุนบริการรถรับส่งตามเกณฑ์ที่กำหนด เพื่อให้ผู้มีความจำเป็นเข้าถึงบริการได้ 2) ดำเนินโครงการจิตอาสาโดยองค์กรปกครองส่วนท้องถิ่นเป็นผู้รับผิดชอบงบประมาณ และสาธารณสุข เป็นผู้สนับสนุนการพัฒนาศักยภาพของจิตอาสาให้สามารถดูแลผู้สูงอายุโรคเรื้อรังได้อย่างมีประสิทธิภาพ และ 3) พัฒนาศักยภาพสถานีอนามัยให้เป็นศูนย์สุขภาพชุมชน เพื่อให้สามารถดูแลผู้สูงอายุโรคเรื้อรัง ในชุมชนได้อย่างมีประสิทธิภาพ

This study aimed to identify roles of the local administrative authority in supporting the continuous healthcare services provided to older people with chronic illness, and to explore expectations of community members on the local administrative authority's roles. The study setting was Bangkrason Subdistrict, of Bangpra-in District, Pranakorn Sriayutthaya Province, and the study participants included 10 older people with chronic illness, 10 caregivers of older people with chronic illness, the local administrative authority, and public health personnel holding management positions. The data was collected using a questionnaire, focus group discussion, and an interview guide. The quantitative data gained then was analyzed using descriptive statistics, while the qualitative data were analyzed using content analysis. The study results revealed that all parties who participated in this study were aware of the problems and effects of chronic illness encountered by the older people and their families. The older people and caregivers tried to solve the problem by adhering to continuous treatment. However, they were unable to keep an appointment at all times as caregivers needed to be at work and the older people were unable to travel alone on public transportation even though the local administrative office had been providing social welfare to older people and low-income people. They provided a vehicle to transport older people to health care facilities in case of emergency. Public health personnel had developed a system to serve older people's needs of continuity of care. The community members expected the local administrative office to provide a vehicle to transport older people to the hospital. They also suggested that the health center should upgrade its capacity to match that of the hospital in order to increase accessibility for clients. This study suggested the establishment of a committee, which is responsible for managing and supporting the continuity of care provided to older people with chronic illness, wherein: 1) the local administrative authority supplies a vehicle for ensuring accessibility to required healthcare of older people with chronic illness who meet the pre-determined criteria; 2) a "Volunteer Project" is established-the local administrative authority would provide financial support, while the public health sector would organize training to increase volunteers' competency for taking care of older people with chronic illness; and 3) the health center is upgraded to a primary care unit in order to provide effective healthcare services for older people with chronic illness.
