การประเมินความต้องการจําเป็นในการบริการดูแลสุขภาพผู้สูงอายุของโรงพยาบาลส่งเสริมสุขภาพตําบล

Abstract

วัตถุประสงค์:เพื่อประเมิน จัดอันดับ และเปรียบเทียบความต้องการจําเป็นในการบริการดูแลสุขภาพผู้สูงอายุของโรงพยาบาลส่งเสริมสุขภาพตําบล (รพ.สต.) รูปแบบการวิจัย:การวิจัยเชิงพรรณนา วิธีดําเนินการวิจัย: กลุ่มตัวอย่าง ได้แก่ รพ.สต. จังหวัดกาญจนบุรี จํานวน 104 แห่ง การเก็บรวบรวมข้อมูลจากผู้ให้ข้อมูลของแต่ละโรงพยาบาลใช้วิธีส่งแบบสอบถามทางไปรษณีย์ วิเคราะห์ข้อมูลด้วยสถิติพรรณนา ค่า modified priorityneeds index (PNI Modified) และการวิเคราะห์ความแปรปรวน ผลการวิจัย: หมวดการให้บริการในครอบครัว/ชุมชนของผู้สูงอายุ ได้รับการประเมินจากกลุ่มตัวอย่างว่า เป็นความต้องการจําเป็นมากที่สุดในการบริการดูแลสุขภาพผู้สูงอายุ รองลงมา คือ หมวดการเชื่อมโยงส่งต่อผู้สูงอายุที่มีปัญหา หมวดการให้บริการในรพ.สต. และหมวดการส่งเสริมสุขภาพและการป้องกันโรค ตามลําดับ รพ.สต. ทั้งขนาดเล็ก ขนาดกลาง และขนาดใหญ่ จัดลําดับความสําคัญหมวดการให้บริการในครอบครัว/ชุมชนเป็นอันดับหนึ่ง ส่วนหมวดการเชื่อมโยงส่งต่อผู้สูงอายุที่มีปัญหา ได้รับการประเมินของรพ.สต. ขนาดเล็กและขนาดกลาง ให้มีความสําคัญเป็นอันดับสอง ในขณะที่รพ.สต.ขนาดใหญ่ ประเมินให้หมวดการให้บริการในรพ.สต. มีความสําคัญเป็นอันดับสอง และไม่พบความแตกต่างกันอย่างมีนัยสําคัญทางสถิติในแต่ละหมวดและในภาพรวมทุกหมวดของความต้องการจําเป็นในการให้บริการดูแลสุขภาพผู้สูงอายุระหว่างรพ.สต. ที่มีขนาดความรับผิดชอบแตกต่างกัน (p > .05) สรุปและข้อเสนอแนะ: การดําเนินงานของรพ.สต. ด้านการให้บริการในครอบครัว/ชุมชนของผู้สูงอายุ รวมทั้งการจัดสรรบุคลากรและอุปกรณ์การแพทย์อย่างเพียงพอในการดําเนินงาน ควรได้รับการส่งเสริมและสนับสนุนจากรัฐบาลกระทรวงที่เกี่ยวข้อง และองค์การปกครองส่วนท้องถิ่น

Purpose: To assess, prioritize and compare needs of health care services for older adults in sub-district health promoting hospitals.Design: Descriptive research.Methods: The study sample included 104 sub-district health promoting hospitals in KanchanaburiProvince. Data were collected from a key informant from each hospital using mailed questionnaires.Descriptive statistics, modified priority needs index (PNI Modified) and analysis of variance were usedfor data analysis.Main findings: Among four dimensions of health care services, the dimension of family andcommunity care services was rated as the most essential need for older adults, followed by the referraland network system, the outpatient care services, and the health promotion and prevention, respectively.All three hospital sizes rated the essential need for the family and community care services as the firstpriority of health services for older adults. The second ranking belonged to the referral and networksystem rated by the hospitals with small and medium sizes, while the outpatient care services rated bythe hospital with large size. The essential needs for each dimension and the entire aspect of health careservices were not significantly different among the hospitals with different sizes (p > .05).Conclusion and recommendations: The family and community care services provided bysub-district health promoting hospitals should be received more support from the government, relatedministry, and local administration. Such support should also cover sufficient human resources andmedical equipment for the services provided.
