สถานะองค์ความรู้ นโยบาย กลไกการดำเนินงาน และระบบวิจัย เพื่อการแก้ไขปัญหาการตั้งครรภ์ในวัยรุ่น : ข้อเสนอเชิงยุทธศาสตร์

Abstract

บทความ มีวัตถุประสงค์เพื่อ 1)ศึกษาสถานะองค์ความรู้ ระบบข้อมูล มาตรการ กลวิธีด้านการป้องกันและแก้ไขปัญหาการตั้งครรภ์ในวัยรุ่น 2) ศึกษาปัญหาและข้อเสนอของระบบวิจัยเพื่อการแก้ไขปัญหาการตั้งครรภ์ในวัยรุ่น ระเบียบวิธีการศึกษา เป็นทบทวนองค์ความรู้จากงานวิจัยที่เกี่ยวข้องกับการตั้งครรภ์ในวัยรุ่นในประเทศไทย ระหว่างปี พศ. 2556-2562 และ การสัมภาษณ์เชิงลึกกับผู้ทรงวุฒิทางวิชาการ ผู้จัดการงานวิจัย ผู้ปฎิบัติงานในองค์กรพัฒนาเอกชน และรัฐ จำนวน 8 คน การวิเคราะห์ข้อมูล เป็นการวิเคราะห์ เชิงเนื้อหาจากเอกสารงานวิจัยและบทสัมภาษณ์ องค์ความรู้ที่ผ่านมาพบว่า การตั้งครรภ์ในวัยรุ่นเกี่ยวข้องกับเหตุปัจจัยที่มีมีความซับซ้อน(complexity) และเกี่ยวโยงกับตัวกำหนดทางสังคมด้านสุขภาพหลายมิติ และความเปราะบางทางสังคมของวัยรุ่น การศึกษาวิจัยส่วนใหญ่พบว่าอยู่ภายใต้กรอบแนวคิดเชิงพฤติกรรม และการพัฒนาโปรแกรมที่มุ่งการแก้ปัญหาเชิงพฤติกรรม สอดคล้องกับงานของต่างประเทศที่มีข้อวิพากษ์ในประเด็นที่ว่าองค์ความรู้เรื่องการตั้งครรภ์ในวัยรุ่นที่เกี่ยวข้องกับความไม่เท่าเทียมทางสังคมยังมีน้อย องค์ความรู้ส่วนใหญ่มักเน้นไปในเรื่องการขาดข้อมูล หรือการเข้าถึงบริการการคุมกำเนิดเป็นปัจจัยกำหนดการตั้งครรภ์ในวัยรุ่นยังน้อย ขณะที่องค์ความรู้ในต่างประเทศ ชี้นำกรอบการมองปัญหาการตั้งครรภ์ในวัยรุ่น ไปในการมองปัญหาเชิงโครงสร้างที่เป็นเรื่องความเปราะบางทางสังคม และ ความด้อยโอกาสหลายมิติ (intersectionality) ในปัญหาการตั้งครรรภ์ในวัยรุ่น ที่ซึ่งต้องใช้กรอบการมองจากหลายสาขาวิชา หลายมุมมอง การมีระเบียบวิธีการวิจัย และกลยุทธเพื่อการเปลี่ยนแปลงสังคม ระบบการวิจัยมักเป็นไปเพื่อเป็นการสร้างความรู้ จึงจำเป็นต้องพัฒนาระบบการวิจัยเพื่อเชื่อมโยงกับผู้ใช้ประโยชน์ และให้ความสำคัญกับการเปลี่ยนแปลงระบบกลไกและโครงสร้างเพื่อแก้ไขปัญหาการตั้งครรภ์ในวัยรุ่น บทความนี้จะช่วยให้ผู้อ่านมีความรู้และเข้าใจ เงื่อนไขกำหนดการตั้งครรภ์ในวัยรุ่นที่มีสำคัญ ตลอดจน การเสนอความท้าทายและยุทธศาสตร์ด้านการพัฒนาองค์ความรู้และระบบวิจัย เพื่อการแก้ไขปัญหาการตั้งครรภ์ในวัยรุ่นอย่างมีประสิทธิภาพ

The paper aims to 1) analyze the state of knowledge, information systems, the measures of resolution in regard toteenage pregnancy problems 2) research systems gap and recommendation for research systems regarding teenage pregnancy solutions. Data collection includes 1) a review literature between 2013-2019 related to teenage pregnancy in Thailand together with theoreticallens analysis 2) In-depth interviews with 8 informants including academic experts, research manager and government and non government officers. The content analysis was done from research papers and interviews.Teen pregnancy is determined by the complex, multi level social dimensions which are related to social determinants of health and social vulnerabilities of young people. However, epidemiological information systems still do not reflect all the information that identifies the problem. Most studies have found that it is within the framework of behavioral model that aimed at solving individual level. Most of them tend to focus on the lack knowledge and the limit of access to contraceptive services that determine teenage pregnancy. In line with the international knowledge that has been stated teenage pregnancy, research framework should be related to social inequalityfocusing structural explanation of social vulnerability and multiple disadvantage ( intersectionality). This is required a framework from trans- disciplines, muti- level perspectives, research methods and social change strategies. Research systems tend to be for the knowledge based that is needed tocooperate with the users with focusing to intervene the structural to the change of the mechanism and structure to solve the problem of teen pregnancy. This article serves to promote the understanding of the crucial determinants related to teenage pregnancy in Thailand and to demonstrate the challenges and agenda of research knowledge and research system strategies to tackle teenage pregnancy problem effectively.
