การประเมินโครงการ “คลินิกประเมินสุขภาพผู้สูงอายุแบบองค์รวม”

Abstract

การศึกษาครั้งนี้ มีวัตถุประสงค์เพื่อประเมินโครงการคลินิกประเมินสุขภาพผู้สูงอายุ แบบองค์รวม โดยเปรียบเทียบความพึงพอใจต่อบุคลากร การบริการ เวลา และสถานที่ของผู้สูงอายุที่ เข้ามารับบริการที่คลินิกประเมินสุขภาพผู้สูงอายุแบบองค์รวมและคลินิกผู้สูงอายุทั่วไปในแผนกผู้ป่วยนอก ภาควิชาเวชศาสตร์ครอบครัว โรงพยาบาลรามาธิบดี คลินิกผู้สูงอายุแบ่งเป็น 2 คลินิกย่อย คือ 1) คลินิกผู้สูงอายุแบบทั่วไป และ 2) คลินิกประเมินสุขภาพผู้สูงอายุแบบองค์รวม กลุ่มตัวอย่าง เป็นผู้ป่วยนอกที่มารับบริการที่คลินิกผู้สูงอายุทั้งคลินิกผู้สูงอายุแบบทั่วไปและคลินิกประเมิน สุขภาพแบบองค์รวม จำนวน 150 คน เลือกตัวอย่างแบบเจาะจง เก็บข้อมูลโดยใช้แบบสอบถาม ประกอบด้วย 2 ส่วน คือ ข้อมูลส่วนบุคคลและแบบวัดระดับความพึงพอใจ โดยนำมาประเมิน ระดับความพึงพอใจ 4 ด้าน คือ ด้านบุคลากร การบริการ เวลา สถานที่ ของแพทย์ พยาบาล นัก สังคมสงเคราะห์ และเภสัชกร ผลการวิจัยพบว่า กลุ่มตัวอย่างมีอายุเฉลี่ย 73.40 ปี ส่วนใหญ่ เป็นผู้ป่วยสูงอายุที่มารับบริการที่คลินิกผู้สูงอายุแบบทั่วไป (ร้อยละ 63.3) เป็นเพศหญิง (ร้อย ละ 78) มีบ้านที่อยู่อาศัยและที่ดินเป็นของครอบครัวตนเอง (ร้อยละ 80.7) เกือบครึ่งหนึ่งมี สถานภาพสมรสคู่ (ร้อยละ 44.7) จำนวนบุตรมากกว่า 2 คน (ร้อยละ 48.7) ประมาณ 1 ใน 4 มี การศึกษาระดับปริญญาตรี (ร้อยละ 26.7) ระดับความพึงพอใจของผู้ป่วยสูงอายุที่มารับบริการ ที่คลินิกผู้สูงอายุแบบทั่วไป และผู้ป่วยสูงอายุที่มารับบริการที่คลินิกประเมินสุขภาพผู้สูงอายุแบบ องค์รวม ด้านบุคลากร บริการ เวลา และสถานที่ อยู่ในระดับมาก ผู้ป่วยสูงอายุที่มารับบริการ ที่คลินิกประเมินสุขภาพผู้สูงอายุแบบองค์รวมมีระดับความพึงพอใจด้านการบริการและเวลา มากกว่าผู้ป่วยสูงอายุที่มารับบริการที่คลินิกผู้สูงอายุแบบทั่วไป

The purpose of this descriptive study was to evaluate the “Comprehensive Geriatric Assessment Clinic” by examining the patients’ satisfaction with healthcare service at out-patient clinic, Department of Family Medicine, Ramathibodi Hospital. By purposive sampling, 150 patients and relatives were selected from the geriatric clinic of Department of Family Medicine, which was divided into the Comprehensive Geriatric Assessment Clinic and the General Geriatric Clinic. Each patient was asked to complete the questionnaire by themselves. The questionnaire consisted of the Demographic Data Assessment Form and the Patient Satisfaction Questionnaire. The satisfaction level with services provided by physicians, nurses, social workers, and pharmacologists were measured in four parts (personnel, services, time, and places). Results revealed that most of the sample were older patients from general geriatric clinic (63.3 %), female (78%), and those living in their own home (80.7%). Approximately, 45% of them were married and lived with more than 2 children (48.7%). About one quarter (26.7%) had bachelor's degree. The older patients in the Comprehensive Geriatric Assessment Clinic and the General Geriatric Clinic rated their satisfaction at high level in personnel, services, time, and places. Older patients in the Comprehensive Geriatric Assessment Clinic were more significantly satisfied with services and time than those in the General Geriatrics Clinic.
