การพยาบาลเด็กชายที่เข้ารับการผ่าตัดแก้ไขรูเปิดท่อปัสสาวะต่ำกว่าปกติ

Abstract

Hypospadias เป็นความผิดปกติแต่กำเนิดของรูเปิดท่อปัสสาวะอยู่ต่ำกว่าปกติคือ ไม่ อยู่ที่ปลายองคชาติ เป็นเรื่องที่บิดามารดา/ผู้ปกครองและเด็ก มีความวิตกกังวล ดังนั้น พ่อแม่/ ผู้ปกครองควรนำเด็กเข้าพบแพทย์เพื่อตรวจและขอคำปรึกษา ทำให้เกิดความเข้าใจในความผิด ปกติที่เกิดกับเด็ก ในกรณีที่รูเปิดท่อปัสสาวะอยู่ต่ำกว่าปกติไม่มากและองคชาตไม่มีการโค้งงอ อาจไม่จำเป็นต้องทำผ่าตัด แต่ในเด็กที่มีความผิดปกติมากจนไม่สามารถยืนปัสสาวะได้ อาจเกิด ภาวะติดเชื้อทางเดินปัสสาวะและมีผลทางด้านจิตใจ จำเป็นต้องรักษาโดยศัลยแพทย์ผู้เชี่ยวชาญ เฉพาะทาง (กุมารศัลยแพทย์ หรือศัลยแพทย์ระบบปัสสาวะ) จะเป็นผู้พิจารณาว่าต้องผ่าตัด หรือไม่ เมื่อใด และเลือกวิธีที่เหมาะกับเด็กแต่ละราย เพื่อให้ผลของการผ่าตัดรักษาองคชาตมี ลักษณะใกล้เคียงธรรมชาติมากที่สุด เกิดภาวะแทรกซ้อนน้อยที่สุด ภายใต้การทำงานร่วมกันของ สหสาขาวิชาชีพ ก่อนการผ่าตัดทีมวิสัญญีแพทย์ผู้มีความรู้ความสามารถพร้อมให้การดูแลเด็ก ในการให้ยาระงับความรู้สึกและความปวดหลังผ่าตัด พยาบาลห้องผ่าตัดนอกจากมีหน้าที่เตรียม เครื่องมือเครื่องใช้ของการผ่าตัดให้พร้อมตลอดเวลา ยังมีบทบาทสำคัญในการพยาบาลตลอด เวลาทั้งก่อน ขณะ และหลังผ่าตัด เพื่อให้เด็กสุขสบายรู้สึกอบอุ่น ปลอดภัยไม่เกิดอันตรายใดๆ การผ่าตัดแก้ไขรูเปิดท่อปัสสาวะจะใช้วัสดุอุปกรณ์สายรักษ์จิตซึ่งประดิษฐ์ขึ้นใช้ในหน่วยงาน หลัง การผ่าตัดแก้ไขรูเปิดท่อปัสสาวะเด็กสามารถดำรงชีวิตได้อย่างปกติ

Hypospadias is a congenital defect of the urethra in which the opening of the urethra is on the underside, rather than at the end of the penis. The parents/ guardians and children may feel distressed by this condition. Therefore, the parents/ guardians of child with hypospadias should consult physicians to get understanding this malformation. If the hypospadias is mild, with the opening of the urethra just a little down from normal and with no bending of the penis, no treatment will be needed. However, the treatment is needed when the children with moderate or severe forms of hypospadias as the opening to the penis found further back on the penis, which may have to urinate sitting rather than standing or be prone to urinary tract infections and psychological problems. The surgical treatment will be considered by the pediatric surgeon or urological surgeon. If the surgery is performed, the surgeon will select the appropriate operative techniques for each child with hypospadias. The goal of treatment is for normal urination with least complications. The multidisciplinary team approach is vital during the surgical treatment. Before surgery, a child will receive general anesthesia and administer nerve blocks near the penis or in the back to minimize discomfort when the child awakens after the operation. The operative nurses are responsible for not only preparing equipment for surgery, but also providing care during preoperative, intraoperative, and postoperative care to promote safty and comfort of children. The “Sairugjit” stent or catheter will be applied for surgical technique. Succesful surgery will bring the child with hypospadia back to a normal life.
