ความสัมพันธ์ระหว่างปัจจัยคัดสรร และพฤติกรรมการใช้ยาของผู้ป่วยสูงอายุโรคเบาหวาน โรงพยาบาลพัฒนานิคม จังหวัดลพบุรี

Abstract

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาความสัมพันธ์ระหว่างปัจจัยคัดสรร และพฤติกรรมการใช้ยาของผู้สูงอายุ โรคเบาหวาน โรงพยาบาลพัฒนานิคม จังหวัดลพบุรี กลุ่มตัวอย่างเป็นผู้สูงอายุจำนวน 200 คน ที่มารับการรักษาในช่วง ระยะเวลาตั้งแต่เดือนกรกฎาคม-สิงหาคม 2552 รวบรวมข้อมูลโดยใช้แบบสอบถามที่สร้างขึ้นตามแนวคิดแบบแผนความ เชื่อด้านสุขภาพ วิเคราะห์ข้อมูล โดยการแจกแจงความถี่ ค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และค่าไค-สแควร์ ผลการศึกษาพบว่า กลุ่มผู้สูงอายุโรคเบาหวานที่มารับบริการที่โรงพยาบาลพัฒนานิคม ส่วนใหญ่เป็นเพศหญิง มีอายุอยู่ ในช่วง 60-70 ปี สำเร็จการศึกษาชั้นประถมศึกษาปีที่ 4 สถานภาพสมรสคู่ ไม่ได้ประกอบอาชีพ มีรายได้น้อยกว่า 500 บาทต่อเดือน ระยะเวลาที่ป่วยเป็นโรคเบาหวานอยู่ในช่วง 10-20 ปี การใช้ยารักษาโรคอื่นร่วมกับยาเบาหวานของผู้สูงอายุ ได้แก่ ยาลดความดันโลหิตและยาลดไขมันในเลือด และข้อจำกัดที่เกี่ยวข้องกับการใช้ยาที่พบมากที่สุด คือ สายตาพร่ามัว การวิเคราะห์แบบแผนความเชื่อด้านสุขภาพเกี่ยวกับการใช้ยาของผู้สูงอายุโรคเบาหวานที่มารับบริการที่โรงพยาบาล พัฒนานิคม พบว่าส่วนใหญ่มีแบบแผนความเชื่อด้านสุขภาพอยู่ในระดับสูง ผู้สูงอายุกลุ่มตัวอย่างส่วนใหญ่ได้รับการ สนับสนุนทางสังคมอยู่ในระดับปานกลาง และยังมีพฤติกรรมการใช้ยาที่ไม่เหมาะสม การวิเคราะห์ความสัมพันธ์ระหว่าง ตัวแปรที่ศึกษา พบว่า ปัจจัยด้านระยะเวลาการเป็นโรคเบาหวาน การใช้ยารักษาโรคอื่นร่วมกับยาเบาหวาน ข้อจำกัดของ ผู้สูงอายุ และแบบแผนความเชื่อด้านสุขภาพเกี่ยวกับการใช้ยา มีความสัมพันธ์กับพฤติกรรมการใช้ยาของผู้สูงอายุโรค เบาหวานที่มารับบริการที่โรงพยาบาลพัฒนานิคม อย่างมีนัยสำคัญทางสถิติ ข้อเสนอแนะ ได้แก่ การพัฒนารูปแบบ ฉลากยา สำหรับผู้ป่วยเบาหวานที่มีปัญหาด้านสายตา และผู้ป่วยที่อ่านหนังสือไม่ออก ให้มีความเข้าใจและสามารถใช้ ยาได้ถูกต้อง มีการติดตามตรวจสอบพฤติกรรมการใช้ยาของผู้สูงอายุ โดยการเยี่ยมบ้าน

This study aimed to examine the association between selected factors and medication use behavior among older adults with diabetes in Pattananikhom Hospital, Lopburi Province. Two hundred older adults with diabetes were invited to complete a set of questionnaire regarding health belief model of medication use and medication use behavior. Data collection had been conducted between July to August 2009. Data analyses were done by using frequency, distributions of percentage, average mean, standard deviation, and Chi-square Test. The results show that majority of older adults participating in the study were female, age between 60 to 70 years, completed 4 years of primary school, and did not work. Most of them had monthly income less than 500 baht, and were diagnosed as of diabetic for 10-20 years. Other medications they use are medications for hypertension and dyslipidemia. The most physical limitation to manage medication themselves was their visual problems. Their health beliefs regarding medication use was at a high level. Most of them received social support at a moderate level but still performed inappropriate medication use behavior. Association between studied variables revealed that time of being diagnosed, other medication usage together with diabetes, physical limitations, and health belief regarding medication use revealed associations with medication use behavior. Recommendations are to develop special medication instruction for people who have visual problem or who cannot read, and to provide a home visiting schedule to follow-up the medication use behavior assessment.
