ผลของการส่งเสริมบทบาทของย่ายายในการสนับสนุนการเลี้ยงลูกด้วยนมแม่ของมารดาครรภ์แรก

Abstract

วัตถุประสงค์: ย่ายายเป็นบุคคลหนึ่งที่มีอิทธิพลต่อการส่งเสริมการเลี้ยงลูกด้วยนมแม่ การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาผลของการส่งเสริมบทบาทของย่ายายในการสนับสนุนการเลี้ยงลูกด้วยนมแม่ของมารดาครรภ์แรก รูปแบบการวิจัย:การวิจัยกึ่งทดลอง วิธีดําเนินการวิจัย: กลุ่มตัวอย่างคือย่ายายและมารดาหลังคลอดครรภ์แรกที่คลอดปกติ และกําลังพักฟื้นที่โรงพยาบาลไทรน้อย จังหวัดนนทบุรี เลือกกลุ่มตัวอย่างแบบสะดวกจํานวน 50 คน แบ่งเป็นกลุ่มควบคุม 25 คน กลุ่มทดลอง 25 คน ย่ายายกลุ่มควบคุมได้รับการพยาบาลตามปกติ ส่วนย่ายายกลุ่มทดลองได้เข้าร่วมโปรแกรมส่งเสริมบทบาทของย่ายายในการสนับสนุนการเลี้ยงลูกด้วยนมแม่ โดยกิจกรรมในโปรแกรมช่วยพัฒนาความรู้และทักษะให้แก่ย่ายายเพื่อสนับสนุนการเลี้ยงลูกด้วยนมแม่ให้สําเร็จ เก็บรวบรวมข้อมูลโดยใช้แบบสัมภาษณ์ข้อมูลส่วนบุคคล แบบสอบถามการรับรู้ของมารดาต่อการสนับสนุนการเลี้ยงลูกด้วยนมแม่ของย่ายาย แบบสอบถามความคิดเห็นของมารดาต่อโปรแกรมส่งเสริมบทบาทของย่ายายในการสนับสนุนการเลี้ยงลูกด้วยนมแม่ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและการทดสอบค่าที ผลการวิจัย: ผลการศึกษาพบว่า การสนับสนุนการเลี้ยงลูกด้วยนมแม่ของย่ายายตามการรับรู้ของมารดา ในกลุ่มทดลองดีกว่ากลุ่มควบคุมอย่างมีนัยสําคัญทางสถิติ (t = -15.74, p < .001) ยิ่งกว่านั้นมารดาทั้งหมดของย่ายายกลุ่มทดลองมีความพอใจที่ย่ายายได้เข้าร่วมโปรแกรม สรุปและข้อเสนอแนะ:จากผลการวิจัยครั้งนี้พบว่า โปรแกรมส่งเสริมบทบาทของย่ายายในการสนับสนุนการเลี้ยงลูกด้วยนมแม่ มีประสิทธิผลดีในแง่ของการให้ความรู้และการปฏิบัติที่ถูกต้องแก่ย่ายายในการสนับสนุนการเลี้ยงลูกด้วยนมแม่พยาบาลและเจ้าหน้าที่ที่เกี่ยวข้องควรประยุกต์โปรแกรมนี้ไปใช้เพื่อส่งเสริมการเลี้ยงลูกด้วยนมแม่ โดยอาจปรับเปลี่ยนให้เหมาะสมตามบริบทของสถานบริการสุขภาพ

Purpose: Grandmothers are one of the influential persons in breastfeeding promotion. The studyaimed at investigating an effect of a program promoting grandmothers’ role in providing breastfeedingsupport for first-time mothers.Design: Quasi-experimental study.Methods: The study subjects included 50 dyads of grandmothers and first-time mothers afternormal delivery at SaiNoi Hospital, Nontaburi Province, Thailand. A convenience sampling was used to recruit the subjects who were divided into an experimental and a control groups, with 25 each. Thecontrol group received only routine nursing care from the hospital, while the experimental groupparticipated in the program of promoting grandmother’s role in providing breastfeeding support. Theprogram helped equip the grandmothers with knowledge and skill of supporting mothers how to breastfeed successfully. Data were collected by using a demographic information interviewing form, themothers’ perception towards grandparents’ breastfeeding support questionnaire, and the mothers’opinion towards the program questionnaire. Descriptive statistics and independent t-test were used in a data analysis.Main findings: The study findings revealed that grandmothers in the experimental groupprovided better breastfeeding support than those in the control group (t = -15.74, p < .001) as perceivedby the mothers. Moreover, all of the experimental mothers were satisfied with their grandmothers’participation in the program.Conclusion and recommendations: Based on the study findings, the program of promotinggrandmothers’ role in providing breastfeeding support was effective. It is recommended that nurses andother health personnel should apply this program for breastfeeding promotion with certain tailoring to their setting context
