ผลของโปรแกรมการเตรียมความพร้อมในการย้ายออกจากไอ ซี ยู ในผู้ป่วยผ่าตัดทําทางเบี่ยงหลอดเลือดหัวใจและผ่าตัดเปลี่ยน/ ซ่อมลิ้นหัวใจ ต่อความวิตกกังวลจากการย้ายและความพึงพอใจ

Abstract

วัตถุประสงค์: เพื่อศึกษาผลของการใช้โปรแกรมการเตรียมความพร้อมในการย้ายออกจากไอซียูต่อการลดความวิตกกังวลจากการย้ายออกจากไอซียู และความพึงพอใจในการบริการพยาบาล ของผู้ป่วยหลังผ่าตัดทําทางเบี่ยงหลอดเลือดหัวใจ และผ่าตัดเปลี่ยน/ซ่อมลิ้นหัวใจรูปแบบการวิจัย: การศึกษากึ่งทดลองวิธีการดําเนินการวิจัย: กลุ่มตัวอย่าง 60 คน เป็นผู้ป่วยอายุ 18 ปีขึ้นไป ที่ได้รับการผ่าตัดทําทางเบี่ยงหลอดเลือดหัวใจและผ่าตัดเปลี่ยน/ซ่อมลิ้นหัวใจ โรงพยาบาลศิริราช จํานวน 60 ราย คัดเลือกกลุ่มตัวอย่างตามเกณฑ์ที่กําหนด แบ่งเป็นกลุ่มควบคุมซึ่งได้รับการดูแลแบบปกติ และกลุ่มทดลองซึ่งได้รับโปรแกรมการเตรียมความพร้อมในการย้ายออกจากไอซียู จํานวนกลุ่มละ 30 คน เก็บรวบรวมข้อมูลด้วย แบบวัดความวิตกกังวลก่อนย้ายออกจากไอซียู แบบวัดความวิตกกังวลหลังย้ายออกจากไอซียู และแบบสอบถามความคิดเห็นผู้ใช้บริการต่อคุณภาพการบริการพยาบาล วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา การทดสอบไคสแควร์ และการทดสอบทีผลการวิจัย: กลุ่มทดลองมีความวิตกกังวลน้อยกว่ากลุ่มควบคุมทั้งก่อนและหลังย้ายออกจากไอซียูอย่างมีนัยสําคัญ (p < .05) และกลุ่มทดลองมีความพึงพอใจต่อการบริการพยาบาลมากกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างไม่มีนัยสําคัญทางสถิติ (p > .05) สรุปและข้อเสนอแนะ: โปรแกรมการเตรียมความพร้อมผู้ป่วยในการย้ายออกจากไอซียู หลังผ่าตัดทําทางเบี่ยงหลอดเลือดหัวใจ และผ่าตัดเปลี่ยน/ซ่อมลิ้นหัวใจ สามารถลดความวิตกกังวลของผู้ป่วยก่อนและหลังย้ายออกจากไอซียูได้ จึงควรส่งเสริมให้พยาบาลใช้โปรแกรมนี้ในการดูแลผู้ป่วยกลุ่มนี้

Purpose: This research aimed to determine the effect of the ICU transferred-preparation programfor coronary artery bypass graft and replacement/repair valvular patients on transfer anxiety and theirsatisfaction towards the nursing services.

Designs: Quasi-experimental design.

Methods: A sample of 60 patients admitted to the cardiovascular thoracic ICU at Siriraj Hospitalwith coronary artery bypass graft and valvular heart replacement/repair was recruited. The sample wasdivided into a control group receiving usual care and an experimental group receiving the ICUtransferred-preparation program with 30 patients each. The instruments for data collection wereconsisted of a demographic characteristics data from, Pre-Transfer Anxiety Inventory (Pre-TAI), Post-Transfer Anxiety Inventory (Post -TAI) and satisfaction questionnaires. The data were analyzed by usingfrequency, percentage, mean, standard deviation, Chi-square test, and t-test.

Main finding: The results showed that both transfer anxiety scores prior to and after thetransferred periods in the experimental group was significantly lower than that in the control group (p <.05). However, the satisfaction scores in the experimental group was not significantly higher than that inthe control group (p > .05).

Conclusion and recommendations: The results suggest that using the ICU transferred –preparation program for patients with coronary artery bypass graft and valvular heart replacement/repair helps improve quality of care and decrease the anxiety of patients who were transferred from ICU.The program should be encouraged for use in a routine nursing practice with this group of patients.
