การสำรวจงานวิจัยทางการพยาบาลแบบไม่ทดลองที่ศึกษาเกี่ยวกับผู้ป่วยเบาหวานในประเทศไทย

Abstract

การศึกษาครั้งนี้เป็นการสำรวจงานวิจัยทางการพยาบาลแบบไม่ทดลองที่ศึกษาเกี่ยว กับผู้ป่วยเบาหวานในประเทศไทย ตั้งแต่ปี พ.ศ. 2523 จนถึงปี พ.ศ. 2548 ทั้งงานวิจัยที่เป็น วิทยานิพนธ์และงานวิจัยที่ไม่ได้เป็นส่วนหนึ่งของการศึกษาซึ่งมีการตีพิมพ์ในวารสารทางการ พยาบาล การแพทย์ และสาธารณสุขในประเทศไทย โดยค้นชื่อเรื่องงานวิจัยจากฐานข้อมูลของ มหาวิทยาลัย ฐานข้อมูลสถาบันพระบรมราชชนก และฐานข้อมูล Thai Index Medicus ผลการ สำรวจ ลักษณะทางกายภาพของงานวิจัย พบว่ามีงานวิจัยจำนวน 230 ชื่อเรื่อง ส่วนใหญ่ผลิตใน ช่วงปี พ.ศ. 2540-2548 เป็นประเภทวิทยานิพนธ์ระดับปริญญาโท ใช้วิธีการวิจัยเชิงบรรยาย ศึกษาในกลุ่มตัวอย่างที่เป็นผู้ป่วยเบาหวานซึ่งมารับการรักษาที่คลินิกโรคเบาหวาน กลุ่มตัวอย่าง ได้มาจากการเลือกแบบเจาะจง ตัวแปรหลักที่ใช้ในการศึกษาส่วนใหญ่คือ ตัวแปรด้านภาวะการ ควบคุมโรค และตัวแปรด้านพฤติกรรมสุขภาพ งานวิจัยส่วนใหญ่มีการแสดงกรอบแนวคิดของ การวิจัย โดยเฉพาะที่เป็นวิทยานิพนธ์ เครื่องมือวิจัยที่ใช้ส่วนใหญ่เป็นแบบสัมภาษณ์ซึ่งสร้าง โดยผู้วิจัย ผลการวิจัยพบว่า มีข้อค้นพบจากการทดสอบสมมติฐานการวิจัย ซึ่งสามารถจำแนก ออกเป็น 3 หมวด ดังนี้ 1) ปัจจัยที่มีความสัมพันธ์กับภาวะการควบคุมโรค 2) ปัจจัยที่มีความ สัมพันธ์กับพฤติกรรมสุขภาพ นอกจากนี้ ยังมีผลการวิจัยที่เป็นข้อมูลเชิงคุณภาพจากงานวิจัย จำนวน 15 ชื่อเรื่อง ซึ่งสรุปไว้ในหมวดที่ 3 การสรุปผลการวิจัยทั้งหมดนี้ทำให้เข้าใจพฤติกรรม สุขภาพของผู้ป่วยเบาหวาน และปัจจัยด้านต่างๆที่มีความสัมพันธ์กับพฤติกรรมสุขภาพเหล่านั้น และยังพบว่าพฤติกรรมสุขภาพของผู้ป่วยมีความสัมพันธ์กับภาวะการควบคุมโรค การสำรวจ งานวิจัยครั้งนี้ได้องค์ความรู้จากงานวิจัยจำนวน 230 ชื่อเรื่อง ซึ่งจะเป็นประโยชน์ต่อพยาบาล และนักวิจัยทางการพยาบาลในการนำไปพัฒนารูปแบบการพยาบาลผู้ป่วยเบาหวานที่มี ประสิทธิภาพในการปรับเปลี่ยนพฤติกรรมสุขภาพให้ควบคุมภาวะโรคเบาหวานได้ดีต่อไป

This study was a survey of existing non-experimental nursing research related to diabetic patients in Thailand from 1980 until 2005. There were two sources of research: theses, and research articles published in nursing, medical, and public health related journals in Thailand. Research titles were retrieved through the library database system of the universities, Praboromarajchanok Library, and Thai Index Medicus database. It was revealed that there were 230 research titles. Most of the studies were disseminated between 1997 to 2005, and were theses for Master’s degree. Descriptive methodology was mostly used to study variables among diabetic patients who had received medical services at diabetes clinics. Most of the samples were recruited using purposive sampling. The main variables found in most of these research titles included diabetic control variables and health behavior variables. Most of the Master’s theses presented conceptual frameworks. The majority of instruments used were interviewing forms developed by the researchers. Results of the hypotheses testing reported in these 230 research titles could be classified into 3 categories: 1) factors related to diabetic control, and 2) factors related to health behaviors. In addition to these, results of qualitative data reported in 15 research titles could be summarized into the third category. The summary in these 3 categories has led to understanding of health behaviors of diabetic patients and other factors related to such behaviors. The findings of this survey also were that patients’ health behaviors are related to their diabetic control. The findings from this survey provide knowledge derived from 230 research titles. Nurses and nurse researchers can use these findings to develop effective interventions to modify patients’ health behaviors aiming at good diabetic control.
