ผลของโปรแกรมส่งเสริมการดูแลผู้ป่วยระยะสุดท้ายต่อความรู้และการรับรู้ สมรรถนะในตนเองของพยาบาลวิชาชีพ

Abstract

การวิจัยครั้งนี้เป็นงานวิจัยกึ่งทดลอง มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมส่งเสริม การดูแลผู้ป่วยระยะสุดท้าย ต่อความรู้และการรับรู้สมรรถนะในตนเองของพยาบาลวิชาชีพ โดยใช้ แนวคิดทฤษฎีการรับรู้สมรรถนะในตนเอง (self-efficacy theory) ของ แบนดูรา เป็นแนวทาง ในการศึกษา กลุ่มตัวอย่างเป็นพยาบาลวิชาชีพที่ปฏิบัติงานในแผนกศัลยกรรม อายุรกรรม กุมารเวชกรรม และสูติ-นารีเวชกรรม โรงพยาบาลตำรวจ เลือกกลุ่มตัวอย่างตามเกณฑ์คุณสมบัติ ที่กำหนด จำนวน 60 ราย เป็นกลุ่มทดลองกลุ่มเดียว วัดซ้ำ 3 ครั้ง กลุ่มตัวอย่างได้รับโปรแกรม ส่งเสริมการดูแลผู้ป่วยระยะสุดท้าย เก็บข้อมูลโดยใช้แบบประเมินความรู้การดูแลผู้ป่วยระยะสุดท้าย และแบบประเมินการรับรู้สมรรถนะในตนเองของพยาบาลในการดูแลผู้ป่วยระยะสุดท้าย ผลการศึกษา พบว่า พยาบาลมีความรู้และการรับรู้สมรรถนะในตนเองในการดูแลผู้ป่วยระยะสุดท้ายหลังสิ้นสุด โปรแกรมทันที และหลังสิ้นสุดโปรแกรม 4 สัปดาห์ สูงกว่าก่อนเข้าโปรแกรมส่งเสริมการดูแลผู้ป่วย ระยะสุดท้าย อย่างมีนัยสำคัญทางสถิติที่ระดับ .001 แต่ความรู้และการรับรู้สมรรถนะในตนเอง ในการดูแลผู้ป่วยระยะสุดท้ายเมื่อเปรียบเทียบระหว่างหลังสิ้นสุดโปรแกรมทันที และหลังสิ้นสุด โปรแกรม 4 สัปดาห์ พบว่า ไม่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ผลการศึกษา ครั้งนี้แสดงให้เห็นว่าโปรแกรมส่งเสริมการดูแลผู้ป่วยระยะสุดท้ายช่วยเพิ่มความรู้และการรับรู้ สมรรถนะในตนเองของพยาบาล นำโปรแกรมดังกล่าวใช้เป็นแนวทางในการส่งเสริมการดูแลผู้ป่วย ระยะสุดท้ายต่อไปได้

The purpose of this quasi-experimental research was to examine the effect of a supportive educational end-of-life care program on knowledge and perceived self-efficacy of professional nurses. This research framework was guided by Bandura’s self-efficacy theory. The participants of this research were nurses working in the departments of surgery, medicine, pediatrics, obstetrics and gynecology in the Police General Hospital, Bangkok. The one group pretest-posttest design was used in this study. A sample of 60 professional nurses was selected by purposive sampling. Each test was conducted in 3 trials. The participants undertook the program, and the results were recorded as knowledge and perceived self-efficacy scores. The result of this study indicated that immediately after finishing the program and at up to 4 weeks after the program, the subjects had greater knowledge and perceived self-efficacy than before entering the program with statistical significance. However, in comparing the scores of knowledge and perceived self-efficacy of the subjects, there was no significant different between the scores on immediately finishing the program and those obtained four weeks later. From the result of this study, it is evident that the supportive educational end-of-life care program promotes greater knowledge and perceived self-efficacy in professional nurses. Therefore, it should be used for prospective supporting end-of-life care.
