การประเมินผลโครงการเอ็นซีดีแอดโฮมเพื่อการรักษาและดูแลผู้ป่วยโรคเบาหวานและความดันโลหิตสูง ในอำเภอวัฒนานคร จังหวัดสระแก้ว

Abstract

การประเมินผลโครงการเอ็นซีดีแอดโฮมเพื่อการรักษาและดูแลผู้ป่วยโรคเบาหวานและความดันโลหิตสูง อำเภอวัฒนานคร จังหวัดสระแก้ว ประยุกต์ใช้โมเดลตรรกะ ประกอบด้วย ปัจจัยนำเข้า กระบวนการ ผลผลิต และผลลัพธ์ ดำเนินการในกลุ่มผู้ปฏิบัติงานโครงการที่ดูแลผู้ป่วย ณ สถานบริการและที่บ้าน กลุ่มตัวอย่างคือผู้ปฏิบัติงานหลักในโครงการ 26 คน เก็บข้อมูลด้วยการสัมภาษณ์เจาะลึก สนทนากลุ่ม และรวบรวมข้อมูลจากเอกสารในประเด็นปัจจัยนำเข้าและกระบวนการ และจากระบบบริการในประเด็นผลผลิตและผลลัพธ์ วิเคราะห์ข้อมูลด้วยการวิเคราะห์เนื้อหา การแจกแจงความถี่ และร้อยละ ผลการวิจัยจากข้อมูลการรับบริการ ด้านผลลัพธ์พบว่าหลังเข้าโครงการสัดส่วนผู้ป่วยเบาหวานที่มีระดับ HbA1c ในเกณฑ์ควบคุมได้ลดลง สัดส่วนผู้ป่วยความดันโลหิตสูงที่มีระดับความดันโลหิตในเกณฑ์ควบคุมได้ใกล้เคียงกับก่อนเข้าโครงการ และไม่พบผู้ป่วยเข้ารักษาซ้ำในแผนกผู้ป่วยใน ด้านผลผลิตพบว่าจำนวนครั้งเข้ารับบริการของผู้ป่วยลดลงเล็กน้อย ส่วนการขาดนัดเพิ่มขึ้นในช่วงหลัง ผลการวิจัยจากเอกสารร่วมกับสัมภาษณ์ผู้ปฏิบัติงานหลักด้านปัจจัยนำเข้าและกระบวนการพบว่าการอบรมฟื้นฟูอาสาสมัครสาธารณสุขประจำหมู่บ้านไม่ต่อเนื่อง เกณฑ์การวินิจฉัยและคัดเลือกช่วยลดผลกระทบในผู้ป่วยแต่จำกัดจำนวนผู้เข้าร่วมโครงการ การใช้แอปพลิเคชันในการดูแลผู้ป่วยลดลง มีการปรับกระบวนการเยี่ยมบ้านและส่งยาถึงบ้านในช่วงการระบาดของโควิด 2019 งบประมาณไม่เพียงพอ และระบบสารสนเทศบางส่วนซ้ำซ้อนกับระบบเดิม

This research aimed to evaluate the NCD@Home Project to manage the treatment and care of diabetes and hypertensive disorder patients in Wattana Nakhon of Sa Kaeo province by applying the Logic Model, which included inputs, processes, outputs, and outcomes. It was carried out among the project workers caring for patients at health facilities and at home. The samples consisted of 26 primary project workers. Data was collected through in-depth interviews, focus group discussions, documents review on inputs and processes, and data retrieved from the service system on outputs and outcomes. Data was analyzed by content analysis, frequency, and percentage. From the service data, the outcomes showed that after participating in the project, the proportion of diabetic patients who possess the HbA1c at the controllable level decreased, the proportion of hypertensive patients whose blood pressure was at the controllable level was similar to before entering the project., and there were no patients repeated visits as in-patients. In terms of outputs, the number of visits slightly decreased while the absence of appointments increased in the latter period. Findings from the documentary review together with interviews with key informants on inputs and processes showed that the refreshers’ courses for village health volunteers were not continuity, diagnostic and selection criteria reduced the impact on patients but limited the number of eligible participants, use of the application has been minimized, home visit and home drug delivery process was reorganized during the COVID-19 pandemic, insufficient budget, and some information systems were redundant.
