การพัฒนาเครื่องมือดึงรั้งเนื้อเยื่อปลายแหลมในการผ่าตัดปลูกถ่ายเนื้อเยื่อ

Abstract

การผ่าตัดปลูกถ่ายเนื้อเยื่อเป็นการผ่าตัดที่ต้องมีการตัดต่อหลอดเลือดและ เส้นประสาท ซึ่งต้องใช้ความระมัดระวังในการแยกเนื้อเยื่อ ปัจจัยที่จะทำให้การตัดต่อหลอดเลือด และเส้นประสาทประสบความสำเร็จได้นั้น ส่วนหนึ่งเกี่ยวข้องกับการดึงรั้งเนื้อเยื่อระหว่างการ ผ่าตัดและรักษาสภาพเนื้อเยื่อก่อนนำไปปลูกถ่าย ปัญหาที่พบ คือ เครื่องมือดึงรั้งเนื้อเยื่อที่นำมา ใช้มีขนาด และความลึก ไม่เหมาะสมกับตำแหน่งที่ทำผ่าตัด ส่งผลให้การผ่าตัดล่าช้าและเนื้อเยื่อ ที่นำมาปลูกถ่ายบอบช้ำจากแรงกด ผู้เขียนจึงเกิดแนวคิดที่จะพัฒนาเครื่องมือดึงรั้งเนื้อเยื่อปลาย แหลมขึ้น โดยมีวัตถุประสงค์เพื่อใช้ในการแยกเนื้อเยื่อชั้นต่างๆ ออกจากหลอดเลือดและเส้น ประสาท ช่วยให้ศัลยแพทย์มองเห็นบริเวณผ่าตัดได้ชัดเจนขึ้น ทำให้การผ่าตัดเป็นไปด้วยความ สะดวกและรวดเร็ว ประเด็นสำคัญ คือ ป้องกันเนื้อเยื่อบอบช้ำจากการผ่าตัด เครื่องมือดึงรั้งเนื้อเยื่อ ปลายแหลมนี้ประดิษฐ์จากเหล็กกล้าไร้สนิม ส่วนปลายด้านหนึ่งมีลักษณะโค้งเป็นขอเกี่ยว ทำให้ ยึดเนื้อเยื่อได้มั่นคงโดยส่วนคมไม่ทำให้เนื้อเยื่อฉีกขาด ปลายด้านตรงข้ามขดเป็นก้นหอยใช้ สำหรับสอดยางวงเพื่อยึดกับตัวหนีบผ้า จากการประเมินความพึงพอใจของศัลยแพทย์และ พยาบาลผู้ใช้เครื่องมือดึงรั้งเนื้อเยื่อ พบว่า ค่าเฉลี่ยความพึงพอใจโดยรวมของผู้ใช้ อยู่ในระดับ พึงพอใจมากที่สุด เครื่องมือดึงรั้งเนื้อเยื่อปลายแหลมนี้สามารถนำไปพัฒนารูปแบบให้เหมาะสม กับการผ่าตัดด้านอื่น และการปรับรูปแบบให้เหมาะสมกับผู้ป่วยทีมี่น้ำหนักตัวมาก เพื่อลดแรงกด ภายนอก

The main operation of free tissue transfer is the revascularization of the vessel at the recipient site. One of the factors instrumental in making a free tissue transfer successful involves retracting tissue and preserving the tissue for transfer. A common problem found during free tissue transfer is that the instruments are not of a suitable size for the recipient site, causing delays in the operation and damage to the transplanted tissue. Therefore, the researchers have developed as fish hook retractor aimed to retract and differentiate the muscle layer from blood vessels and nerves, and to expose the operation field clearly for efficiency. The fish hook retractor is made of stainless steel. It is curved on one end in order to retract and protect the tissue at the donor site and the other end is designed to be held with a towel clip. Results from evaluation by surgeons and scrub nurses who used the fish hook retractor revealed a very high satisfaction level on average. It is suggested that the fish hook retractor can be applied to the other fields of operation and can be modified for overweight patients in order to reduce external pressure.
