In a cohort study of 36 patients with community-onset extended-spectrum β-lactamase (ESBL)-producing Escherichia coli or Klebsiella pneumoniae bloodstream infections, we found that predictors of mortality were community-onset infection with ESBL-producing K. pneumoniae pathogens (P = .02) and failure to receive an initial empirical regimen that included either β-lactam and β-lactamase-inhibitors or a carbapenem (P = .04). © 2008 by The Society for Healthcare Epidemiology of America.
