ปัจจัยที่มีความสัมพันธ์กับการมีส่วนร่วมในการปฏิบัติงานของเจ้าหน้าที่ทีมเฝ้าระวังสอบสวนเคลื่อนที่เร็ว ในกรุงเทพมหานคร

Abstract

การวิจัยเชิงสำรวจภาคตัดขวางนี้ มีวัตถุประสงค์เพื่อศึกษาความสัมพันธ์ระหว่างปัจจัยต่างๆ กับการมีส่วนร่วมในการปฏิบัติงานของเจ้าหน้าที่ทีมเฝ้าระวังสอบสวนเคลื่อนที่เร็ว ในกรุงเทพมหานคร กลุ่มตัวอย่างเป็นเจ้าหน้าที่ทีม SRRT ของศูนย์บริการสาธารณสุข กรุงเทพมหานครจำนวน 68 แห่ง จำนวน 228 คน ใช้วิธีการสุ่มตัวอย่างเป็นระบบ ใช้แบบสอบถามเป็นเครื่องมือในเก็บข้อมูล ในเดือน พฤษภาคม 2560 วิเคราะห์ข้อมูลด้วยความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และค่า สัมประสิทธิ์สหสัมพันธ์แบบเพียร์สัน ผลการศึกษา พบว่า กลุ่มตัวอย่างส่วนใหญ่เป็นเพศหญิง อายุ 42 ปี ระดับการศึกษาสูงสุด ปริญญาตรี สถานภาพสมรส เคยอบรมระบาดวิทยา โดยฝึกอบรมหลักสูตรระบาดวิทยาและการบริหาร จัดการทีมเฝ้าระวังสอบสวนเคลื่อนที่เร็วสำหรับแพทย์หัวหน้าทีมและผู้สอบสวนหลัก (FEMT) อบรม เฉลี่ย 1 ครั้ง ในระยะเวลาเฉลี่ย 2 ปี ตำแหน่งสมาชิกในทีม SRRT ส่วนใหญ่เป็นพยาบาลวิชาชีพ ระยะเวลาปฏิบัติงานเฉลี่ย 6 ปี รายได้มากกว่า 30,000 บาท การมีส่วนร่วมในการปฏิบัติงานระดับสูง ค่าเฉลี่ย 2.50 (จากคะแนนเต็ม 3) ภาวะผู้นำในตัวบุคคลระดับสูง ค่าเฉลี่ย 4.03 (จากคะแนนเต็ม 5) ความรู้การสอบสวนควบคุมโรคติดต่อ (โรคไข้เลือดออก และโรคมือ เท้า ปาก) คะแนนเต็ม 27 ค่าเฉลี่ย 21 การรับรู้การประเมินการปฏิบัติตามนโยบาย ส่วนใหญ่มีการประสานงานกับหน่วยงานอื่นที่เกี่ยวข้อง ระดับสูง ค่าเฉลี่ย 4.33 (จากคะแนนเต็ม 5) สัมประสิทธิ์สหสัมพันธ์ระหว่างปัจจัย ภาวะผู้นำในตัวบุคคล ความรู้การสอบสวนควบคุมโรคติดต่อ มีความสัมพันธ์กับการมีส่วนร่วมในการปฏิบัติงาน ระดับปาน กลาง อย่างมีนัยสำคัญทางสถิติที่ p-value < 0.001 (r = 0.392 และ 0.678) และการรับรู้การประเมิน การปฏิบัติตามนโยบาย มีความสัมพันธ์กับการมีส่วนร่วมในการปฏิบัติงาน ระดับสูง อย่างมีนัยสำคัญ ทางสถิติที่ 0.01 (r = 0.736) ตามลำดับ การวิจัยมีข้อเสนอแนะว่าควรมีการถ่ายทอดนโยบายและการบริหารงานทีม SRRT เป็นลำดับ ขั้นลงสู่ระดับพื้นที่อย่างเป็นระบบไร้รอยต่อ ควรมีความเข้าใจตรงกัน เพื่อสามารถปฏิบัติงานได้ทันเวลา สำเร็จตามเป้าหมาย ควรมีการจัดเวทีสัมมนา แลกเปลี่ยนเรียนรู้อย่างสม่ำเสมอ มีการบริหารจัดการ ภาระงานให้เหมาะสมต่อหน้าที่ที่ปฏิบัติ

This cross-sectional survey research aimed at studying relationships between factors related to the performance and participation of the staff in surveillance and rapid response team (SRRT) in Bangkok. 228 SRRT staff from 68 public health centers in Bangkokwere selected using the systematic random sampling. Data collection tool was questionnaires distributed in May, 2017. Data was analyzed by using frequency distribution, percentage, mean, standard deviation, and Pearson’s correlation coefficient. The research result showed that most samples were female, 42 years of age, married, graduated with a Bachelor’s degree as the highest education, and averagely attended the Field Epidemiology and Management Training (FEMT) at least once in two years. Most SRRT staff held a professional nurse position with 6 years of work experience and earned more than 30,000 Baht averagely. Their work participation was at a high level with mean = 2.50 (from the maximum score of 3). Personal leadership was at a high level with mean = 4.03 (from the maximum score of 5). The mean of knowledge in communicable disease control and investigation (Dengue fever and Hand, Foot, and Mouth disease) was at 21 from the maximum score of 27. Regarding their perception of performance assessment according to the policy, most staff coordinated with other involved sectors at a high level with mean = 4.33 (the maximum score = 5). For the correlation coefficients between factors, their personal leadership and knowledge in communicable disease control and investigation were related to their performance and participation at a moderate level with statistical significance p-value < 0.01 (r = 0.392 and 0.678). Their perception of performance assessment according to the policy was related to their performance and participation at a high level with statistical significance p-value < 0.01 (r = 0.736). It is suggested that policies and SRRT management should be conveyed from the highest authority to local levels systematically and seamlessly. Member of the team should have mutual understanding in order to accomplish tasks in time according to the set goals. Seminar platforms to share and exchange knowledge should be organized regularly. Management of tasks and assignments should also suit the staff’s duties.
