ผลของโปรแกรมการสอนต่อความรู้และพฤติกรรมของผู้ดูแลเพื่อส่งเสริมภาวะโภชนาการในเด็กโรคหัวใจพิการแต่กำเนิดชนิดไม่เขียว

Abstract

วัตถุประสงค์: เพื่อศึกษาผลของโปรแกรมการสอนต่อความรู้และพฤติกรรมของผู้ดูแลเพื่อส่งเสริมภาวะโภชนาการในเด็กโรคหัวใจพิการแต่กำเนิดชนิดไม่เขียวในขวบปีแรก

รูปแบบการวิจัย: การวิจัยกึ่งทดลอง

วิธีดำเนินการวิจัย: กลุ่มตัวอย่างเป็นผู้ดูแลของเด็กโรคหัวใจพิการแต่กำเนิดชนิดไม่เขียว อายุ 6-12 เดือน ที่รอรับการผ่าตัดแก้ไขความผิดปกติอย่างสมบูรณ์ (total repair) มาเข้ารับการรักษาที่โรงพยาบาลมหาวิทยาลัย ระดับตติยภูมิ การเลือกกลุ่มตัวอย่างเป็นแบบสะดวก แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 26 ราย กลุ่มทดลองได้รับความรู้ตามโปรแกรมการสอนที่ผู้วิจัยพัฒนาขึ้น โดยใช้กรอบแนวคิดการพยาบาลระบบสนับสนุนและให้ความรู้ของ โอเร็ม ในการศึกาาครั้งนี้รวบรวมข้อมูลโดยใช้แบบสอบถามความรู้ และแบบสอบถามพฤติกรรมการดูแลให้อาหารสำหรับเด็กโรคหัวใจพิการแต่กำเนิดชนิดไม่เขียว วิเคราะห์ข้อมูลโดยใช้สถิติวิเคราะห์ความแปรปรวนร่วม

ผลการวิจัย : กลุ่มทดลองมีค่าเฉลี่ยของคะแนนความรู้ และพฤติกรรมเพื่อส่งเสริมภาวะโภชนาการในเด็กโรคหัวใจพิการแต่กำเนิดชนิดไม่เขียวสูงกว่ากลุ่มควบคุม อย่างมีนัยสำคัญทางสถิติ (F = 5.288, df = 1, p < .05; F = 4.568, df = 1, p < .05; ตามลำดับ).

สรุปและข้อเสนอแนะ: จากผลการวิจัย แสดงให้เห็นว่าโปรแกรมการสอนมีประสิทธิภาพ ช่วยส่งเสริมความรู้และพฤติกรรมของผู้ดูแล ในการให้อาหารสำหรับเด็กโรคหัวใจพิการแต่กำเนิดชนิดไม่เขียวในขวบปีแรก ทั้งนี้เพื่อให้เด็กได้รับการดูแลที่ถูกต้องเหมาะสม มีภาวะโภชนาการที่ดี ร่างกายแข็งแรง ไม่เกิดภาวะแทรกซ้อนและมีความพร้อมสำหรับการผ่าตัดตามแผนการรักษาต่อไป

Purpose: This study aimed to examine the effect of a teaching program on knowledge and behaviors of caregivers to promote nutrition in children with acyanotic congenital heart disease during first year of life.

Design: Quasi-experimental research design.

Methods: The sample composed of caregivers of children with acyanotic congenital heart disease aged 6-12 months who were waiting for total repair surgery and were treated at a tertiary university hospital. The sample was selected by convenience sampling, and divided into control group and experimental group, with 26 subjects in each group. The experimental group received a teaching program developed by the researcher using Orem’s Educative and Supportive Nursing System as the conceptual framework of this study. Data were collected using the knowledge questionnaire and feeding behaviors questionnaire. Data were analyzed using ANCOVA.

Main findings: The experimental group had statistically significant higher mean score of knowledge and behaviours to promote nutrition in children with acyanotic congenital heart disease than those in the control group (F = 5.288, df = 1, p < .05; F = 4.568, df = 1, p < .05; respectively).

Conclusion and recommendations: The results suggested that the teaching program is effective in increasing caregivers’ knowledge and behaviors to improve the nutrition of children with acyanotic congenital heart disease during their first year of life. This program may assist these children to receive proper care and better nutrition, maintain better health, without complications, and be prepared for surgery according to treatment plans.
