การรับรองมาตรฐานสากลโรงพยาบาลในประเทศไทย : สถานการณ์และแนวโน้ม

Abstract

การรับรองมาตรฐานเป็นกลไกควบคุมคุณภาพที่สถาบันรับรองคุณภาพสถานพยาบาลทั่วโลกนำมาเป็นเครื่องมือเพื่อการประเมิน และส่งเสริมให้การดูแลบริการสุขภาพได้มาตรฐาน บทความนี้มีวัตถุประสงค์เพื่อวิเคราะห์สถานการณ์และแนวโน้มการรับรองมาตรฐานสากลของโรงพยาบาลในไทยตั้งแต่เดือน กุมภาพันธ์ พ.ศ. 2545 ถึง เดือนเมษายน พ.ศ. 2556 จากการศึกษาพบว่ามีโรงพยาบาลผ่านการรับรองมาตรฐานสากล (Joint Commission International Standards) จำนวน 23 แห่ง ทั้งหมดสังกัดภาคเอกชน สถานที่ตั้งอยู่ในเขตภาคกลาง 15 แห่ง (กรุงเทพมหานคร 14 แห่ง และนนทบุรี 1 แห่ง) เขตภาคใต้ 3 แห่ง (ภูเก็ต 2 แห่ง และสมุย 1 แห่ง) เขตภาคตะวันออก 1 แห่ง (ชลบุรี) เขตภาคตะวันตก 1 แห่ง (ประจวบคีรีขันธ์) และเขตภาคเหนือ 2 แห่ง (เชียงใหม่ 1 แห่ง และพิษณุโลก 1 แห่ง) ขนาดโรงพยาบาลมีตั้งแต่ 50 ถึง 538 เตียง พ.ศ. 2545 เป็นปีแรกที่โรงพยาบาลของไทยผ่านการรับรอง 1 แห่ง ต่อมาในปี พ.ศ. 2547 จนถึง พ.ศ. 2556 จำนวนโรงพยาบาลที่ผ่านการรับรองเพิ่มเป็น 4, 5, 9, 13, 15, 22 และ 23 แห่งตามลำดับ ปัจจุบันสถานภาพการรับรองแยกเป็น 2 กลุ่ม คือ กลุ่มที่หนึ่งผ่านการรับรองครั้งแรกจำนวน 13 แห่ง และกลุ่มที่สองผ่านการรับรองซ้ำจำนวน 10 แห่ง การเพิ่มจำนวนโรงพยาบาลจาก 4 แห่ง เป็น 23 แห่งในช่วง 7 ปีนี้ แสดงถึงแนวโน้มโรงพยาบาลเอกชนของไทยได้พัฒนาเข้าสู่มาตรฐานสากลเพิ่มสูงขึ้น Accreditation is a recognized mechanism that the accreditation body used as a tool to enhance quality of care. The objectives of this paper were to analyze the situation and trend of international hospital accreditation in Thailand from February 2002 to April 2013. This study used secondary data and trend analysis for hospitals accredited by Joint Commission International (JCI) from 2002 to 2013. The results were from 23 private hospitals; 15 (14 in Bangkok and 1 in Nonthaburi) located in central region; 3 (2 in Phuket and 1 in Samui Island) located southern region; 1 (in Chonburi) located in eastern region; 1 (in Prachuapkhirikhan) located in western region; and 2 (1 in Chiengmai and 1 in Phitsanulok) located in northern region. The size of hospitals varied from 50 to 538 beds. In 2002, only one hospital was accredited by JCI. The number of accredited hospitals increased to 4, 5, 9, 13, 15, 22 and 23 respectively from 2007 to 2013. The accreditation status were divided into 2 groups. The fi rst group was the initial accreditation status with a total number of 13 hospitals. The second group was the re-accreditation status with a total number of 10 hospitals. The increased number from 4 to 23 showed the trend of development among Thai private hospitals toward international standard.
