กรณีศึกษาการดูแลผู้ป่วยมะเร็งปอดระยะลุกลามแบบประคับประคอง: บทบาทของผู้ปฏิบัติการพยาบาลขั้นสูง

Abstract

ผู้ป่วยมะเร็งปอดระยะลุกลามต้องเผชิญกับอาการ ผลกระทบจากการรักษาและความ ก้าวหน้าของโรคอย่างหลีกเลี่ยงไม่ได้ แนวทางการดูแลรักษาจึงมุ่งหวังเพื่อประคับประคอง ไม่ให้การดำเนินโรคลุกลามเร็ว หรือเพื่อบรรเทาอาการที่ก่อให้เกิดความทุกข์ทรมาน ช่วยให้ผู้ป่วย และครอบครัวมีคุณภาพชีวิตที่ดีตามบริบท บทความนี้นำเสนอบทบาทของผู้ปฏิบัติการพยาบาล ขั้นสูง ในการดูแลผู้ป่วยมะเร็งปอดระยะลุกลามแบบประคับประคอง ตั้งแต่ระยะเริ่มต้นของการ รักษา โดยใช้หลักสมรรถนะการดูแลผู้ป่วยระยะท้ายและสมรรถนะของผู้ปฏิบัติการพยาบาลขั้นสูง ที่กำหนดโดยสภาการพยาบาลมาประยุกต์ใช้ร่วมกับแนวคิดของการดูแลแบบประคับประคอง มี วัตถุประสงค์เพื่อวิเคราะห์ประเด็นปัญหาสุขภาพของผู้ป่วย นำความรู้จากหลักฐานเชิงประจักษ์ มาประยุกต์ใช้ในการดูแลช่วยเหลือในแต่ละระยะตามภาวะเบี่ยงเบนทางสุขภาพ และเสนอแนวทาง ในการดูแลผู้ป่วยมะเร็งปอดระยะลุกลามที่เหมาะสม

During their illness trajectory, patients with advanced stage lung cancer encounter physical symptoms, adverse effects from medical treatments, and their disease progression inevitably. The care approach should focus on palliation of their physical symptoms that cause sufferings and on delaying disease progression in order to optimize quality of life for both patients and their families. This article demonstrates the roles of advanced practice nurses in providing palliative care for advanced lung cancer patients since the beginning of their medical care. The principles of palliative care and competencies of advanced practice nurses enacted by Thailand Nursing and Midwifery Council (TNMC) were applied in the care approach to analyze health problems of the patient. Evidence-based nursing care was implemented in the care relying on the disease stage and health deviation self-care. Finally, palliative nursing care for patients with advanced lung cancer was proposed.
