การพัฒนางานบริการบำบัดระยะสั้นผู้ป่วยโรคข้ออักเสบ หน่วยบำบัดระยะสั้นโรคข้อ โรงพยาบาลรามาธิบดี

Abstract

การวิจัยพัฒนาเชิงทดลอง แบบกลุ่มเดียว วัด ก่อน-หลัง การทดลองนี้ เพื่อพัฒนางานบริการบำบัดระยะสั้นผู้ป่วยโรคข้ออักเสบให้มีประสิทธิภาพมากขึ้น สิ่งที่ใช้ในการทดลอง คือ รูปแบบการดำเนิินงานบริการบำบัดระยะสั้นผู้ป่วยโรคข้ออักเสบที่พัฒนาขึ้น นำไปทดลองที่หน่วยโรคภูมิแพ้อิมมูโนวิทยาและโรคข้อ โรงพยาบาลรามาธิบดี ระหว่าง 1 กรกฎาคม พ.ศ. 2554 ถึง 30 มิถุนายน พ.ศ. 2555 ใช้เวลา 1 ปี กลุ่มตัวอย่าง คือ "งาน" การให้บริการบำบัดระยะสั้นผู้ป่วยโรคข้ออักเสบ ตั้งแต่ผู้รับบริการมาถึง จนกระทั่งกลับออกไป ในช่วง 1 เดือน ก่อนการทดลอง และในช่วง 1 เดือน หลังการทดลอง รวมทั้งสิ้น 66 ครั้ง ผู้ตอบแบบสอบถาม คือ ผู้บริหาร 2 คน ผู้ปฏิบัติงาน 6 คน และผู้รับบริการ 66 คน เปรียบเทียบผลการดำเนินงาน ระหว่างก่อน กับ หลัง การทดลอง ในด้านปริมาณงาน คุณภาพงาน ระยะเวลาและแรงงานที่ใช้ในการให้บริการ ความพึงพอใจของผู้เกี่ยวข้อง และต้นทุนต่อหน่วยในการให้บริการ ด้วยค่าสถิติพรรณนา ค่าสถิติ-ที, Mann Whitney U และ Wilcoxon Match-Paired Signed-Ranks ที่ระดับแอลฟ่า 0.05 พบว่า หลังการทดลอง อัตราความถูกต้องและความครบถ้วนของการปฏิบัติตามมาตรฐานกิจกรรมการให้บริการผู้ป่วยโรคข้ออักเสบเพิ่มขึ้น (P < 0.05) ผลการดำเนินงานทั้ง 5 ด้านดีขึ้น (P < 0.05) รูปแบบใหม่ที่พัฒนาขึ้นมีองค์ประกอบที่ครบถ้วนทั้งด้านหลักการ โครงสร้าง และวิธีการนำรูปแบบไปใช้ โดยใช้เพียงทรัพยากรเท่าที่มีอยู่ ด้วยกระบวนการพัฒนาที่ไม่ยุ่งยาก สามารถนำไปประยุกต์ในการพัฒนางานทั้งหลายได้อย่างมีประสิทธิภาพ เป็นตัวอย่างของการทำงานประจำให้เป็นผลงานวิจัย เน้นการมีส่วนร่วมของผู้ปฏิบัติงานทุกระดัับในการพัฒนาตั้งแต่เริ่มต้น

The experimental development research, one group pre-test and post-test design aimed to improve the efficiency of the short stay service for rheumatologic patients. The experimental intervention was a working model, developed by researcher and implemented at Ramathibodi Hospital for 1 year during July 1, 2011 - June 30, 2012. The samples were 66 services, start from patient entering to discharge, during 1 month of pre-test and 1 month of post-test. The respondents for satisfaction questionnaires were 2 executive officers, 6 staffs, and 66 patients. Compared the results between pre-test and post-test in terms of quantity, quality, times and labor-forces consumed, satisfactions of involving person, and unit cost of the service using statistical t, MannWhitney U and Wilcoxon Match-Paired Signed-Ranks at alpha 0.05. The results revealed that after experimentation, the accuracy and completeness rate of the working by standard operating procedure was increased (P < 0.05), all of the 5 working performances were better (P < 0.05). The new developed model composed of complete elements in term of principles, structures, and implementing methods using only the existing resources by appropriated and not complicate method in developing the model. It can be applied to other working improvement efficiently to be a good example of routine to research emphasizing on staffs participation in every level from the beginning.
