ปัจจัยทำนายการให้บริการช่วยเลิกบุหรี่ของทันตแพทย์

Abstract

ทันตแพทย์สามารถช่วยผู้รับบริการทันตกรรมที่สูบบุหรี่ให้เลิกบุหรี่ได้ การวิจัยเชิงสํารวจภาคตัดขวางครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาการให้บริการช่วยเลิกบุหรี่ และปัจจัยทํานายการให้บริการช่วยเลิกบุหรี่ตามกรอบแนวคิด PRECEDE-PROCEED model ตัวอย่างคือ ทันตแพทย์ที่ปฏิบัติงานในโรงพยาบาลระดับทุติยภูมิไม่เกิน 120 เตียง สังกัดกระทรวงสาธารณสุข จําานวน 539 คน เก็บข้อมูลด้วยแบบสอบถามแบบตอบด้วยตนเองทางไปรษณีย์ และออนไลน์ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสถิติเชิงอนุมาน ผลการวิจัย พบทันตแพทย์ช่วยเลิกบุหรี่สม่ําเสมอสูงสุด ได้แก่ การแนะนําเลิกบุหรี่ ร้อยละ 50.3 เมื่อพิจารณาการให้บริการช่วยเลิกบุหรี่ครบ 2 ขั้นตอน(2As) สม่ําเสมอ พบทันตแพทย์ปฏิบัติ ร้อยละ 23.0 โดยมี 8 ปัจจัยที่มีความสัมพันธ์กับการช่วยเลิกบุหรี่ 2As สม่ําเสมออย่างมีนัยสําคัญทางสถิติ ได้แก่ ปีที่สําเร็จการศึกษา การรับรู้ความสามารถตนเอง การรับรู้บทบาทหน้าที่ ความตั้งใจในการช่วยเลิกบุหรี่ คลินิกช่วยเลิกบุหรี่ นโยบายช่วยเลิกบุหรี่ระดับจังหวัดและระดับหน่วยงาน การได้รับปัจจัยเสริมแรง โดยความตั้งใจในการช่วยเลิกบุหรี่เป็นปัจจัยทํานายสําาคัญที่สุดต่อการให้บริการช่วยเลิกบุหรี่ 2As สม่ําเสมอของทันตแพทย์ ดังนั้น การจัดทําแนวทางให้บริการเลิกบุหรี่ครบ 2 ขั้นตอน รวมการส่งต่อ จึงควรได้รับการพัฒนาเพื่อให้เหมาะสมกับบริบทของทันตแพทย์

Dentists can have a hand in helping patients quit smoking. The cross-sectional survey research aimed to describe the smoking cessation service provided by dentists and factors predicting smoking cessation services in the concept of PRECEDE-PROCEED model. The samples were 539 dentists who worked in the secondary care level hospitals, not more than 120 beds under the Ministry of Public Health. Data were collected by self-administered postal and electronic questionnaires. Data were analyzed using descriptive statistics and inferential statistics. The results showed 50.3% of dentists had regularly advised their smoking patients to stop smoking. With regard to 2As smoking cessation services, 23.0% of dentists regularly implemented 2As service. Eight factors were signifi cantly related to 2As smoking cessation service composing of year of graduation, self-effi cacy, role perception, intention to provide smoking cessation services, quit smoking clinic, provincial policy, dental setting policy, and reinforcement factors. The intention to provide smoking cessation services could signifi cantly predict 2As smoking cessation service. Therefore, the 2As smoking cessation guidelines including referral service should be developed in accordance with the context of dental practices.
