Study and analysis of relation factors causing healthcare workers in Chulabhorn Hospital by needlestick and sharps injuries

Abstract

Most of the procedures performed by healthcare workers in hospitals involve the use of needlestick and sharps injuries. For this reason, healthcare workers are likely to get the accidents with needlestick and sharps injuries than other occupations. This results in a high risk of infection from blood and secretions. Therefore, the objective of this research was to study and explain the factors including personnel, equipment and environment that affect needlestick and sharps injuries of the healthcare workers in Chulabhorn hospital. This research was a descriptive study. 301 healthcare workers were recruited from 7 positions in Chulabhorn hospital. Data were collected by using a questionnaire (Google Form) and chi-square was used to test the hypothesis. Results and data were analyzed with the SPSS version 18. This research found that most of the positions that were pierced by needlestick and sharp injuries were nurses. The most place for accidents was the inpatient ward. In addition, most of the accidents occurred on the left index finger. The factors, namely job position, gender, age, work experience and education level did not affect the needlestick and sharp injuries accident in Chulabhorn hospital. The result indicated that the type of sharp did not affect the accident location of the needlestick and sharp injuries as well. However, this study revealed that personnel equipment and environmental factors were very related important to work. In conclusion, the factors related to personnel, equipment and environment were important to prevent needlestick and sharps injuries. Therefore, enough supply of equipment for work, good working environment, organizing projects to reduce stress and promoting leisure and health care policies were encouraged to reduce the risk of accidents in healthcare workers as much as possible. Implication of the thematic paper: The hospital can use the information obtained from the study to manage the provision of equipment. Personnel health promotion arrangements of a place, lighting, or suitable working conditions should be taken into account to reduce the risk of accidents with needlesticks and sharp injuries.

การทำหัตถการของบุคลากรทางการแพทย์ในการดูแลผู้ป่วยในโรงพยาบาลส่วนใหญ่เกี่ยวข้องกับการใช้เข็มหรือของมีคม ทำให้บุคลากรทางการแพทย์มีโอกาสเสี่ยงต่อการได้รับอุบัติเหตุเข็มทิ่มตำและของมีคมบาดมากกว่าอาชีพอื่น ๆ ส่งผลให้มีความเสี่ยงสูงต่อการติดเชื้อจากเลือดและสารคัดหลั่ง ดังนั้นการศึกษานี้มีวัตถุประสงค์เพื่อศึกษาและอธิบายปัจจัยที่เป็นความสัมพันธ์ด้านบุคคล อุปกรณ์ และสิ่งแวดล้อมที่มีผลต่อการเกิดอุบัติเหตุเข็มทิ่มตำและของมีคมบาดในบุคลากรทางการแพทย์ โรงพยาบาลจุฬาภรณ์ ใช้รูปแบบการวิจัยเชิงพรรณนา กลุ่มตัวอย่างเป็นบุคลากรทางการแพทย์ 7 ตำแหน่งงานในโรงพยาบาลจุฬาภรณ์ จำนวน 301 คน เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามผ่านการตอบจาก Google Form และดำเนินการวิเคราะห์ข้อมูลโดยใช้สถิติ Chi-square test ในการทดสอบสมมติฐานในการวิจัย ประมวลผล และวิเคราะห์ข้อมูลด้วยโปรแกรมสำเร็จรูป SPSS Version 18 ผลการวิจัยครั้งนี้พบว่า ส่วนใหญ่ตำแหน่งงานที่ถูกเข็มทิ่มตำและของมีคมมากที่สุด คือ พยาบาล เกิดเหตุบ่อยที่สุดที่หอผู้ป่วยใน และเกิดเหตุที่นิ้วชี้ข้างซ้ายมากที่สุด ทั้งนี้ ปัจจัยเรื่องตำแหน่งงาน เพศ อายุ ประสบการณ์ในการทำงาน และระดับการศึกษา ไม่ได้ส่งผลต่อการเกิดอุบัติเหตุเข็มทิ่มตำและของมีคมบาดในโรงพยาบาลจุฬาภรณ์ และชนิดของมีคมไม่ได้มีผลต่อการเกิดตำแหน่งอุบัติเหตุเข็มทิ่มตำและของมีคมบาดเช่นกัน อย่างไรก็ตาม การศึกษาครั้งนี้ทำให้ทราบว่า ปัจจัยด้านบุคคล อุปกรณ์ และสิ่งแวดล้อม เป็นเรื่องที่สำคัญต่อการทำงานอย่างมาก สรุปได้ว่า ปัจจัยด้านบุคคล อุปกรณ์ และสิ่งแวดล้อม เป็นเรื่องที่สำคัญต่อการทำงาน ดังนั้น การจัดหาอุปกรณ์ในการทำงาน การทำให้สภาพแวดล้อมในการทำงานเหมาะสม การจัดโครงการเพื่อลดความเครียด และส่งเสริมนโยบายการพักผ่อนและการดูแลสุขภาพ เพื่อลดความเสี่ยงในการเกิดอุบัติเหตุในบุคลากรทางการแพทย์ได้มากที่สุด การนำผลของสารนิพนธ์ไปใช้: โรงพยาบาลสามารถนำข้อมูลที่ได้จากการศึกษานำไปใช้ในการบริหารจัดการในการจัดหาอุปกรณ์ การส่งเสริมสุขภาพบุคลากร การจัดสถานที่ แสงสว่าง หรือสภาวะการทำงานที่เหมาะสม เพื่อลดความเสี่ยงในการเกิดอุบัติเหตุเข็มทิ่มตำและของมีคมบาด
