การพัฒนางานการดูแลผู้ป่วยใส่เครื่องกระตุ้นหัวใจแบบบูรณาการ สถาบันโรคทรวงอก

Abstract

การวิจัยพัฒนาเชิงทดลอง แบบสองกลุ่ม ทดลอง-ควบคุม วัดผลก่อน-หลัง ครั้งนี้วัตถุประสงค์ เพื่อพัฒนารูปแบบใหม่ในการดูแลแผลผู้ป่วยใส่เครื่องกระตุ้นหัวใจและเพื่อเปรียบเทียบผลการดำเนินงานก่อนและหลังการทดลอง 5 ด้าน คือ ด้านปริมาณ ด้านคุณภาพ ด้านความพึงพอใจ ด้านแรงงานที่ใช้และด้านเศรษฐศาสตร์โดยใช้ผู้ป่วยใส่เครื่องกระตุ้นหัวใจ 42 คนในสถาบันโรคทรวงอกเป็นพื้นที่ทดลอง 24 คนในโรงพยาบาลราชวิถีเป็นพื้นที่ควบคุม ระหว่างวันที่ 1 พฤศจิกายน 2557 - 28 กุมภาพันธ์ 2558 จำนวนผู้ตอบแบบสอบถาม 3 กลุ่มจำนวน 117 คน ผลการ ดำเนินงานใช้สถิติพรรณนา จำนวน ร้อยละ ค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐาน เปรียบเทียบผลการดำเนินงาน กลุ่ม Independent sample ที่ข้อมูลมีการกระจายปกติ ใช้สถิติ Independent t-test หรือ Mann Whitney U test ผลการวิจัยพบว่า ปริมาณงานในการทำแผลผู้ป่วยต่อรายมีปริมาณงานลดลง ( p = 0.008 ) ค่าใช้จ่ายในการทำแผลลดลง (p =0.03 ) ความพึงพอใจเพิ่มขึ้นทั้ง 3 กลุ่ม ( p < 0.05 ) อัตราการติดเชื้อภายใน 72 ชั่วโมง ไม่แตกต่าง (p = 1.00) อัตราแรงงานที่ใช้ในการทำแผล ลดลง(p =0.03 ) สรุปได้ว่า รูปแบบงานการดูแลผู้ป่วยใส่เครื่องกระตุ้นหัวใจที่พัฒนาขึ้นมาใหม่มีประสิทธิภาพมากขึ้น สามารถลดปริมาณงาน ลดค่าใช้จ่ายและความพึงพอใจผู้เกี่ยวข้องเพิ่มขึ้น

This is an experimental development research, pre-test and post-test design to develop a new working model of wound care for patients undergoing PPM, and compare this with a conventional model in workload, quality, satisfaction, labor-force, and cost.The new working model of wound care for patients undergoing PPM was applied to 42 patients in CCIT during 1st November 2014 to 28th February 2015. 24 patients in Rajvithi Hospital were used as the control group. 3 questionnaires were collected from 117 persons involved in this study. Data were analyzed by descriptive statistics, t-test, Mann-whitney U, paired-t, Wilcoxson matched-paired-sign-rank, and content analysis. After the experiment, the workload related to wound care decreased (p=0.008), cost of wound care decreased (p=0.03), satisfaction was improved (p<0.05), wound infection rate was not changed(p=1.00), cost was decreased (p=0.03) Conclusion The new model of PPM patient's care was more efficient. It can increase quality of care, decrease workload, decrease cost and improve the satisfaction
