การพยาบาลที่มีคุณภาพตามมาตรฐานที่กำหนดจะช่วยให้ผู้ป่วยและผู้สูงอายุที่มีภาวะพึ่งพิงมีคุณภาพชีวิตที่ดีขึ้น และการปฏิบัติการพยาบาลเพื่อให้ได้มาตรฐานตามเป้าหมายขึ้นอยู่กับหลายปัจจัย การวิจัยเชิงพรรณนาแบบภาคตัดขวางนี้ มีวัตถุประสงค์เพื่ออธิบายปัจจัยที่มีความสัมพันธ์กับการปฏิบัติงานของพยาบาลเยี่ยมบ้านตามมาตรฐานการดูแลผู้ป่วยและผู้สูงอายุที่ต้องได้รับการดูแลที่บ้านตามกรอบแนวคิดของ McCormick และ Ilgen กลุ่มตัวอย่างเป็นพยาบาลวิชาชีพปฏิบัติงานเยี่ยมบ้านของศูนย์บริการสาธารณสุข สำนักอนามัย กรุงเทพมหานคร จำนวน 251 คน เก็บข้อมูลโดยใช้แบบสอบถามจำนวน 3 ส่วน ได้แก่ แบบสอบถามปัจจัยส่วนบุคคล แบบสอบถามปัจจัยสถานการณ์ด้านองค์กร สังคมและสิ่งแวดล้อม และแบบสอบถามการปฏิบัติงานของพยาบาลเยี่ยมบ้านตามมาตรฐานการพยาบาลผู้ป่วยและผู้สูงอายุที่ต้องได้รับการดูแลที่บ้าน วิเคราะห์ข้อมูลด้วยการแจกแจงความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สถิติไคว์สแควร์ และสถิติถดถอยโลจิสติกส์ทวิผลการศึกษาพบว่า พยาบาลเยี่ยมบ้านมีการปฏิบัติงานตามมาตรฐานการดูแลผู้ป่วยและผู้สูงอายุที่ต้องได้รับการดูแลที่บ้านโดยรวมอยู่ในระดับมาก ปัจจัยที่มีความสัมพันธ์ต่อการปฏิบัติงานของพยาบาลเยี่ยมบ้านตามมาตรฐานการดูแลผู้ป่วยและผู้สูงอายุที่ต้องได้รับการดูแลที่บ้าน ได้แก่ ปัจจัยส่วนบุคคลด้านการรับรู้มาตรฐานการปฏิบัติงาน (p<0.001) และปัจจัยสถานการณ์ด้านนโยบายองค์กร (p=0.026) ด้านการฝึกอบรมเพิ่มเติม (p<0.001) การนิเทศงาน (95%CI=1.014−13.824) และภาระงาน (95%CI=0.015−0.592)ข้อเสนอแนะที่ได้จากผลการศึกษา ควรมีการฝึกอบรมผู้ปฏิบัติงานใหม่ การฟื้นฟูความรู้และทักษะ การนิเทศงาน การกำหนดนโยบายเป็นลายลักษณ์อักษรในเรื่องมาตรการหรือแนวทางในการป้องกันหรือจัดการความเสี่ยงต่าง ๆ รวมทั้งมีแนวทางการจัดสรรอัตรากำลังของพยาบาลเยี่ยมบ้านให้เหมาะสมกับจำนวนประชากรในแต่ละพื้นที่ เพื่อควบคุมภาระงานและลดโอกาสเสี่ยงที่จะปฏิบัติงานไม่ได้ตามมาตรฐาน
Providing quality nursing care in accordance with the established standards will enable the patients and elderly to receive safe care and a better quality of life. A wide range of factors contribute for the nurse practice to reach the target standard. This analytical cross-sectional descriptive study aimed to examine factors associated to home visit nurses performance based on the standard of home care for dependent patients and elderly people at home. By applying the McCormick and Ilgen’s Framework, 251 home visit nurses of the Public Health Center, Health Department, Bangkok Metropolitan were invited into the study. Data collection was performed by using self-administrative questionnaires consisting of 3 parts: personal information, organizational and social and environmental situation, and the home visit nursing practice according to nursing standards for dependent patients and elderly at home. Frequency, percentage, mean, and standard deviation, Chi-square, Correlation and Binary logistic regression statistics were employed for data analyses. Results revealed that the home visit nurse performance was at a high level. Factors associated to home visit nurse performance were personal information of perceived standard of performance (p<0.001), and organizational policy scenario factors (p = 0.026), which comprise additional training (p<0.001), supervision (95% CI=1.014-13.824), and workload (95% CI =0.015-0.592). Recommendations from this study include providing trainings for new home visit nurses, additional trainings for current nurses, supervision, written policy on risk prevention and management, and adequate number of manpower to suit the population in each area to control the workload and reduce the risk of failing to perform the work according to the standards.
