ความสัมพันธ์ระหว่างปัจจัยส่วนบุคคลภาวะสุขภาพความกลัวการหกล้มและพฤติกรรมป้องกันการหกล้มของผู้สูงอายุในชุมชน

Abstract

การศึกษาครั้งนี้เป็นการวิจัยเชิงบรรยายความสัมพันธ์ มีวัตถุประสงค์เพื่อศึกษาความสัมพันธ์ระหว่างปัจจัยส่วนบุคคล ภาวะสุขภาพ ความกลัวการหกล้ม และพฤติกรรมป้องกันการหกล้มของผู้สูงอายุที่อาศัยในชุมชนโดยใช้กรอบแนวคิดแบบแผนความเชื่อด้านสุขภาพ กลุ่มตัวอย่าง คือ ผู้สูงอายุ ที่มีประวัติเคยหกล้มในช่วงระยะเวลา 6 เดือนที่ผ่านมา อาศัยอยู่ในชุมชนภายใต้การดูแลของศูนย์บริการสาธารณสุข 16 ลุมพินี เขตปทุมวัน กทม.จำนวน 130 คนเครื่องมือที่ใช้เป็นแบบสัมภาษณ์ เพื่อเก็บรวบรวมข้อมูลเกี่ยวกับปัจจัยส่วนบุคคล ภาวะสุขภาพ ความกลัวการหกล้ม และพฤติกรรมป้องกันการหกล้มของผู้สูงอายุ ในชุมชน วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา สัมประสิทธิ์สหสัมพันธ์เพียร์สัน และสัมประสิทธิ์สหสัมพันธ์สเปียร์แมน ผลการวิจัย พบว่า ผู้สูงอายุในชุมชนที่เคยหกล้ม ในระยะเวลา 6 เดือนที่ผ่านมา มีความกลัวการหกล้มในระดับสูง (Mean = 27.28, SD = 7.93) และมีพฤติกรรมป้องกันการหกล้มในระดับสูง (Mean = 85.07, SD = 9.56) อายุมีความสัมพันธ์ทางบวกกับพฤติกรรมป้องกันการหกล้มของผู้สูงอายุอย่างมีนัยสำคัญทางสถิติ (p = 0.179, p < 0.05) และความกลัวการหกล้ม มีความสัมพันธ์ทางบวกกับพฤติกรรมป้องกันการหกล้ม อย่างมีนัยสำคัญทางสถิติ (r = 0.408, p < 0.01) ระดับการศึกษา อาชีพ รายได้ และ ภาวะสุขภาพ ไม่มีความสัมพันธ์กับพฤติกรรมป้องกันการหกล้มพยาบาล ควรส่งเสริมพฤติกรรมป้องกันการหกล้มของผู้สูงอายุตั้งแต่ผู้สูงอายุวัยต้น และควรประเมินความกลัวการหกล้มเพื่อส่งเสริมพฤติกรรมป้องกันการหกล้มของผู้สูงอายุ

The purpose of this research was to study the relationships between personal factors, health status, fear of falling and fall preventive behaviors of community dwelling older persons. The Health Belief Model of Rosenstock was used. The population of the study includes older persons who were at least 60 years of age with a history of falling over the previous 6 months and were living in the communities, Pathum Wan District, Bangkok. The sampling group included 130 older persons. The instruments included 4 questionnaires assessing personal data, health status, fear of falling and fall preventive behaviors of community dwelling older persons. The data were analyzed using descriptive statistics, Spearman correlation and Pearson's Product Moment Correlation. The results revealed that the older persons who had experienced a fall had a fear of falling at a high level (Mean = 27.28, SD = 7.93) and their fall preventive behaviors were at a high level (Mean = 85.07, SD = 9.56). The positive relationships were found between age and fall preventive behaviors (p = 0.179, p < 0.05). There was also a significant positive relationship between fear of falling and fall preventive behaviors (r = 0.408, p < 0.01). Socioeconomic status and health status did not have a correlation with fall preventive behaviors. Nurses should begin to focus on promoting fall preventive behaviors of community dwelling older persons from an early age. In addition, they should assess the fear of falling because they affect the behavior preventing falls in the elderly.
