Abstract
The purpose of this study was to investigate the risk factors of isoniazid monoresistance among Thai pulmonary tuberculosis patients of Central Chest Institute of Thailand. Data were collected from interview, medical record review, and saliva sample collection from 18th September 2017 to 20th February 2018. The total number of participants was 167, comprising 50 isoniazid mono-resistant (cases) and 117 drug susceptible tuberculosis (controls) patients. Descriptive statistics, Chi-squared test, and multivariate logistic regression analysis were used for data analysis. The results showed that rapid acetylator was less likely to develop isoniazid monoresistance compared with slow acetylator (OR 1.169 95% CI 0.435-3.140), even though the percentage of rapid acetylators which resisted to only isoniazid (16.0%) was higher than that of controls (14.5%). In contrast, the highest percentage of NAT2 phenotype among cases was slow acetylator status (44.0%), whereas that of controls was intermediate acetylator status (51.3%). However, NAT2 *4/*6A was mostly found in both cases (24.0%) and controls (32.5%). In addition, adults who were less than 50 years old were more likely to have isoniazid mono-resistance (aOR 2.281 95% CI 1.101 - 4.728) than elders who were more than 50 years old. Due to the fact that the number of the studies associated with risk factors of isoniazid mono-resistance among Thai TB patients was less, the researchers should be encouraged to do more similar research to prevent this situation in Thailand.
การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยเสี่ยงต่อการดื้อยาไอโซไนอาซิดในผู้ป่วยวัณโรคปอดชาวไทยของสถาบันโรคทรวงอก โดยเก็บข้อมูลจากการสัมภาษณ์, การทบทวนเวชระเบียน และการเก็บตัวอย่างน้ำลายของผู้เข้าร่วมงานวิจัย ระหว่างวันที่ 18 กันยายน 2560 - 20 กุมภาพันธ์ 2561 จำนวน 167 รายจำแนกเป็นผู้ป่วยที่ดื้อยาไอโซไนอาซิดตัวเดียว 50 ราย (กลุ่มกรณีศึกษา) และผู้ป่วยที่ไวต่อยาต้านวัณโรคทุกตัว 117 ราย (กลุ่มควบคุม) วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและสถิติเชิงอนุมาน คือ การทดสอบไคสแควร์ และสถิติการถดถอยพหุแบบลอจิสติคผลการศึกษาพบว่า ผู้ที่มีฟีโนไทป์ แบบ rapid acetylator มีโอกาสดื้อยาไอโซไนอาซิดตัวเดียวน้อยกว่าผู้ที่มีฟีโนไทป์ แบบ slow acetylator (OR 1.169; 95% CI 0.435-3.140) แม้ว่าอัตราส่วนของผู้ที่ดื้อยาไอโซไนอาซิดตัวเดียวที่มีฟีโนไทป์แบบ rapid acetylator (ร้อยละ 16.0) จะมีมากกว่าอัตราส่วนของผู้ที่ไวต่อ ยาต้านวัณโรคทุกตัว (ร้อยละ 14.5) แต่อัตราส่วนฟีโนไทป์แบบ slow acetylator (ร้อยละ 44.0) และแบบ intermediate acetylator (ร้อยละ 51.3) กลับพบมากที่สุดในกลุ่มกรณีศึกษาและกลุ่มควบคุม ตามลำดับ นอกจากนี้จีโนไทป์ แบบ NAT2 *4/*6A กลับพบมากที่สุดในผู้ป่วยทั้งสองกลุ่ม คิดเป็นร้อยละ 24.0 และ 32.5 ตามลำดับ นอกจากนั้น ผู้ป่วยที่อายุน้อยกว่าหรือเท่ากับ 50 ปี มีความสัมพันธ์ต่อการดื้อยาไอโซไนอาซิดตัวเดียวเป็น 2.281 เท่า ของผู้ที่มีอายุมากกว่า 50 ปี (95% CI = 1.101 - 4.728) เนื่องจากการศึกษาเรื่องปัจจัยเสี่ยงของการดื้อยาไอโซไนอาซิดในผู้ป่วยวัณโรคปอดชาวไทยยังมีจำนวนน้อย นักวิจัยควรให้ความสนใจและศึกษาเพิ่มขึ้นเพื่อป้องกันการเกิดดื้อยาไอโซไนอาซิด ในประเทศไทย
